Statement of Work (SOW) / Performance Work Statement (PWS)

To provide scheduled and unscheduled maintenance, and periodic preventive maintenance inspections for specific Intra-Aortic Balloon Pump (IABP) systems located at the Rocky Mountain Regional VA Medical Center in Aurora, CO.

• Furnish labor, parts, supplies, and equipment for maintenance of Teleflex AC3 Optimus IABP systems (specific serial and MX numbers provided).

• Perform annual performance/preventative maintenance according to OEM specifications.

• Provide all scheduled and non-scheduled replacement parts, labor, and travel at no charge.

• Include hardware and software updates, upgrades, and revisions.

• Offer on-site service with priority response within one business day.

• Provide 24/7/365 telephonic technical and service support.

• Expedited shipping for down systems (Level 1: First Overnight) and next business-day morning for non-down systems.

• Offer discounts on IABP rentals, test equipment, and biomedical engineer training.

• Perform scheduled maintenance annually by December 31st, including cleaning, diagnostics, calibration, lubrication, and replacement of worn parts.

• Perform unscheduled maintenance to repair equipment to original manufacturer specifications.

• Equipment shall be operable and available for use 95% of the normal hours of coverage.

• Downtime will be computed from notification of a problem during normal coverage hours.

• Remedies for failure to meet uptime commitment include a reduction in the annual charge.

• Base Year: 07/10/2026 – 07/09/2027

• Option Year 1: 07/10/2027 – 07/09/2028

• Option Year 2: 07/10/2028 – 07/09/2029

• Location: Rocky Mountain Regional VA Medical Center, 1700 N Wheeling Street, Aurora, CO 80045.

• Contractor personnel must be fully qualified FSEs with factory training and a minimum of two years of experience.

• Provide copies of all factory certificates for FSEs.

• Adhere to VA regulations regarding identification, parking, smoking, and firearms.

• Comply with strict Federal laws, regulations, and VA Directives regarding information and information system security, including background investigations and data protection.

• All contractor-supplied test equipment must be properly calibrated and certified, traceable to a national standard.

• Contractor must manage Federal records in accordance with applicable laws and NARA policies.

• Invoices to be submitted monthly in arrears via email to COR, then uploaded to Tungsten upon approval.

• Vendors have 90 days after the period of performance to submit all remaining invoices.
