Statement of Work (SOW) / Performance Work Statement (PWS)

To ensure the continuous reliability of the Wilkes Barre VA Medical Center's two (2) Teleflex AC3 Series Intra-Aortic Balloon Pumps (IABPs), which are critical cardiac support devices used for patients with impaired left ventricular function.

• Provide full service and maintenance, including preventive and corrective maintenance, for the specified Teleflex IABP units.

• Offer 24/7 technical telephone support and onsite support.

• Equipment to be serviced includes two Teleflex AC3 Optimus IABP units (Serial Numbers: 2501265F and 2503056F).

• Services must be performed in accordance with Section D Conformance Standards, including NFPA-99, JC, NEC, OSHA, HIPAA, and Federal/VA security specifications.

• Equipment must be maintained to meet or exceed OEM technical specifications.

• Documentation of all maintenance, including downtime and parts used, is required via an Engineering Service Report (ESR).

• Uncorrected deficiencies or downtime may lead to contract termination.

• Performance Period: Base year plus two option years.

• Place of Performance: Wilkes Barre VA Medical Center, 1111 East End Blvd, Wilkes Barre, PA, 18711.

• Contractor must have an established business with an office and full-time staff, including "fully qualified" Field Service Engineers (FSEs) with recent training and a minimum of two years of experience.

• FSEs must provide proof of service key dongle ownership and TB/flu vaccination documentation, and HIPAA compliance training.

• Compliance with OSHA Bloodborne Pathogens Standard and VHA Directive 1192 (Influenza Prevention Program) is mandatory.

• Contractor must obtain and make available all necessary service manuals and tools.

• All parts supplied must be OEM and compatible; refurbished parts are not permitted.

• Normal hours of coverage are M-F, 8:00 AM to 4:30 PM ET, excluding holidays. Services outside these hours require pre-approval or direction from the CO/POC.

• Customer service must respond to service requests 24/7.

• Deliverables include draft and final copies of documentation within specified timeframes.
