Submission Requirements

- A detailed description of the firm's experience servicing the listed equipment.
- Unique Entity Identifier (UEI) number.
- Name and phone number of a point of contact.
- Mandatory Requirements:
- A letter from the Original Equipment Manufacturer (OEM), Steris Corporation, confirming the vendor is an authorized distributor (per clause 852.212-72(c)).
- Proof of ability to purchase OEM parts, if needed.
- Proof that all technical personnel are OEM certified.
