Statement of Work (SOW) / Performance Work Statement (PWS)

To provide preventive maintenance inspections and intervening service calls for VA San Diego Healthcare System owned Belimed equipment.

• Provide all labor, supervision, materials, parts, and transportation for maintenance and repair of Belimed equipment.

• Services include planned maintenance inspections and intervening service calls.

• Maintain equipment in functional condition according to international standards and original equipment manufacturer (OEM) requirements.

• Perform cleaning, software review, calibration, lubrication, and replacement of faulty/worn parts, electrical wiring, and mechanical components.

• Provide telephone support 24/7 (excluding holidays) and on-site corrective maintenance within 48 hours.

• All wear-parts, service kits, and materials for maintenance and repair are included.

• Equipment performance to be checked according to OEM requirements.

• On-site work to be performed during normal working hours (Monday-Friday, 6:00 AM - 6:00 PM).

• Field service reports to be provided electronically to the COR within 5 business days of on-site visit, detailing services, parts, hours, safety checks, performance data, and recommendations.

• Service technicians must report unsafe conditions or signs of misuse.

• Preventive Maintenance Inspections (PMIs) to be scheduled at least 3 days in advance with the COR and performed during the first 5 working days of specified months.

• Base Year: June 1, 2026, to March 31, 2027 (10 Months)

• Option Year 1: April 1, 2027, to March 31, 2028

• Option Year 2: April 1, 2028, to March 31, 2029

• Option Year 3: April 1, 2029, to March 31, 2030

• Vendor must provide replacement parts meeting or exceeding OEM specifications.

• Hard drives with sensitive information must be disposed of according to facility policy and ISO concurrence.

• Operating and consumable supplies are not included.

• Unserviceable parts to be replaced on an exchange basis within 8 working hours.

• Vendor must have sufficient parts in stock or be able to obtain them.

• Vendor must possess complete service documentation, manuals, and schematics.

• VA will not furnish parts or test equipment.

• Vendor personnel must wear I.D. badges and park in designated areas.

• Vendor must check in/out with Police Service and COR for on-site work.

• Services performed outside normal working hours require a separate, pre-authorized Purchase Order.

• Invoices must be itemized and submitted electronically to the Financial Services Center (FSC).

Solicitation / Intent to Sole Source

This document is a Notice of Intent to Sole Source for maintenance and repair services for Belimed Sterilizers and Washers at the VA San Diego Healthcare Services (VASDHCS) campuses. Justification for Other Than Full and Open Competition

• No Small Disadvantaged Veteran-Owned Small Business (SDVOSB) or Veteran-Owned Small Business (VOSB) vendors were found capable of providing the service.

• A notice of intent to sole source was posted (July 13, 2020, under solicitation 36C26220R0158), yielding no responses.

• Advanced Sterilization Products Service, Inc. was identified as the only capable source.
