Statement of Work (SOW) / Performance Work Statement (PWS)

To provide preventive maintenance and repair services for a government-owned mobile MRI trailer.

• Furnish labor, material, and equipment for preventive maintenance and repairs of the AMST model AMGV-152 mobile MRI trailer.

• Work includes adjustments, calibrations, lubrication, cleaning, testing, and replacement of worn or defective non-consumable parts to maintain the equipment in good operable condition.

• Specific components to be maintained include: HVAC/Chiller system, Lift gate, Roller door, Magnetic Resonance (MR) door, Emergency generator/Automatic Transfer Switch (ATS), Electrical system, and Miscellaneous components.

• All work must be accomplished in accordance with manufacturer's instructions.

• Quarterly and Annual preventive maintenance checks are required as detailed in Appendix A.

• Service reports must be submitted upon completion of work, detailing services performed, parts used, and recommendations.

• Routine repairs are to be performed within the next normal workday after contact.

• Emergency call-back service must be provided within one (1) hour of request, with on-site arrival within 24 hours, at no additional cost.

• Location: Daniel K. Akaka VA Clinic Mobile MRI Trailer, 91-1031 Franklin D. Roosevelt Ave. Kapolei, HI 96707.

• Maintenance contractor shall be escorted by a member of the Facilities Management and Engineering Services Dept at all times.

• Contractor shall furnish all lubricants, cleaning supplies, and tools.

• Contractor must obtain all necessary licenses and permits.

• Contractor is responsible for protecting persons and property and must maintain liability insurance.

• Contractor must coordinate any system shutdowns with VA Facilities Management/Engineering Service staff.

• Contractor personnel must meet security requirements, including background investigations and PIV card issuance, as detailed in the "Contractor Personnel Security Requirements" section.

Sources Sought Announcement

This is a Sources Sought Announcement for Mobile MRI Trailer Preventive Maintenance and Repair services for the VA Pacific Islands Health Care System.
