Statement of Work (SOW) / Performance Work Statement (PWS)

To provide Preventative Maintenance (PM) Services for Artemis Biopsy Ultrasound equipment for the Department of Veterans Affairs Northern California Health Care System (VA NCHCS).

• Perform semi-annual preventative maintenance on specified Artemis Biopsy Ultrasound equipment (MX1663581SN: UAMS0027).

• Services include PM required parts, system verification, alignment, and adjustments.

• PM services are to be performed in accordance with manufacturer's recommended specifications and frequency, and relevant conformance standards (NFPA-99, TJC, NEC, OSHA, HIPAA, Federal and VA security specifications).

• Software updates are to be provided at no cost; software upgrades will be quoted and performed upon COR approval.

• Contractor is responsible for obtaining and maintaining all necessary operational and technical documentation, including service manuals and diagnostic software.

• Services must ensure equipment functions in conformance with the latest requirements of applicable standards and meets or exceeds OEM performance specifications.

• Documentation must include equipment downtime and detailed descriptions of maintenance performed, including replaced parts and costs (for services outside normal hours).

• An Engineering Service Report (ESR) must be completed for each service instance, detailing work performed, parts used, and costs.

• Period of Performance: One base year with four (4) one-year options. Specific dates TBD at award.

• Location: VA NCHCS, Mather, CA.

• Personnel: Field Service Engineers (FSEs) must be "fully qualified" with formalized training and a minimum of two years of experience on the specified equipment. Contractor must provide written assurance of personnel competency and credentials.

• Hours of Coverage: Normal hours are Monday-Friday, 7:30 am to 4:00 pm, excluding holidays. Work outside these hours requires COR approval and may incur separate charges.

• Parts: Contractor shall furnish parts necessary for PM at no cost. Other replacement parts require COR approval and will be quoted.

• Compliance: Adherence to OSHA Bloodborne Pathogens Standard is required, including employee education, risk reduction policies, and provision of appropriate personal protective equipment.

• Documentation: ESRs must be submitted to the COR and include specific details about the service performed, equipment, and costs. Failure to comply with reporting requirements may lead to contract termination.

• Reporting Defects: Contractor must immediately notify the CO and COR of any defects or repairs not covered under the contract and provide a cost estimate.
