Bidders must complete the 'UNIT PRICE' and 'TOTAL AMOUNT' fields for CLIN 0001. This completed schedule will form a critical part of the proposal submission and will be used by the government for evaluation and contract award.

Statement of Work (SOW) / Performance Work Statement (PWS)

To provide comprehensive full-service maintenance, routine, and emergency repair services for government-owned sterilizers, washers, dryers, and other related equipment at the Salt Lake City VA Medical Center.

• Maintenance & Repair: Ensure all equipment functions in conformance with original manufacturer specifications and relevant industry standards (NFPA-99, UL, OSHA, VA manuals).

• Scheduled Maintenance: Perform semi-annual preventive maintenance (PM) on all equipment, including cleaning, diagnostics, calibration, lubrication, remedial maintenance, parts replacement, electrical/mechanical inspections, and annual chamber cleaning for steam sterilizers. PM procedures and schedule to be provided within two weeks of award.

• Unscheduled Maintenance (Emergency Repairs): Provide 24/7 priority technical support and remote monitoring. Field Service Engineers (FSEs) must respond within 30 minutes by phone and arrive on-site within 4 hours if the issue cannot be resolved remotely.

• Parts: Furnish and replace parts as needed to meet uptime requirements. Parts must be equal to or exceed OEM specifications; rebuilt or used parts are prohibited.

• Tools & Manuals: Supply all necessary tools, test equipment, service manuals, and diagnostic software.

• Operational Uptime: Equipment must be operable and available 95% of normal operating hours (8:00 AM - 5:00 PM, Monday-Friday, excluding national holidays). Downtime is computed from notification during normal work hours. Uptime is computed monthly. Failure to meet this may subject the contractor to default actions.

• Response Times: 30-minute phone response for emergencies, 4-hour on-site arrival for unresolved issues.

• Location: Salt Lake City VA Medical Center.

• Service Hours: Routine service/repairs during normal working hours (8:00 AM - 5:00 PM, M-F, excluding holidays), unless requested otherwise by the COR. Emergency repairs are 24/7.

• Compliance: Adherence to NFPA-99, UL, OSHA, and VA manuals.

• Parts Quality: Use only new parts equal to or exceeding OEM specifications.

• Documentation: Detailed Equipment Service Reports (ESR) for all services, including parts and labor. Submit ESRs for acceptance signature and email a copy to the COR.

• Reporting: FSEs must sign in at the Boiler Plant upon arrival and proceed directly to the service area. Document services rendered and obtain an acceptance signature.

A detailed list of equipment requiring service is provided, including ultrasonic cleaners, drying cabinets, disinfectors, sterilizers, warm cabinets, prep and pack tables, washers, and transfer carriages, along with the number of annual inspections required for each.

This is a combined synopsis/solicitation (RFQ 36C25926Q0547) for Steris Sterilization Equipment Preventive Maintenance (PM) and Repair Services for the George E. Wahlen VA Medical Center in Salt Lake City, Utah. This announcement constitutes the only solicitation; no separate written solicitation will be issued.
