Statement of Work (SOW) / Performance Work Statement (PWS)

This Performance Work Statement (PWS) outlines the requirements for a non-personal service contract to provide Licensed Optometrist services for the Navajo Area Indian Health Service (NAIHS) at the Kayenta Health Center (KHC).

• Provide comprehensive eye care to patients in the KHC Eye Clinic.

• Diagnose and manage visual and ocular pathologies.

• Treat visual disorders with corrective lenses, vision training, and pharmacological means.

• Utilize topical and oral ophthalmic therapeutic medicines.

• Provide emergency eye care services.

• Coordinate medical eye care with other physicians.

• Consult with ophthalmologists and other practitioners for complex cases.

• Refer patients for medical, surgical, or diagnostic services outside the Kayenta Service Unit.

• Follow KSU Optometry policies and procedures and Kayenta clinical practice guidelines.

• Adhere to federal licensing, accreditation, and regulating body standards.

• Interact professionally with patients and co-workers.

• Precept optometry students as needed.

• Provide delegated supervision of Eye Clinic personnel.

• Place of Performance: Kayenta Health Center.

• Period of Performance: Flexible, allowing for short-term (13 weeks), long-term (1-3 years), and intermittent support.

• Performance will be evaluated based on standards in the contract and a Performance-based Matrix (Section 8.0).

• Key indicators include Competency, Compliance, Patient Outcomes, Professionalism, Credentialing, Documentation, and Service Quality.

• Satisfactory service will result in payment; incomplete work will not be paid.

• Performance evaluations will be used for future contract selections.

• Contractor must have one year of recent Optometry experience.

• Must possess a current optometry license (diagnostic and therapeutic) and a DEA license.

• BLS for Healthcare providers certification is required.

• Specific immunizations or proof of immunity are required (Rubella, Mumps, Measles, Hepatitis B, Chicken Pox, Tetanus Diphtheria, TB test).

• Must read, understand, speak, and write English and be culturally aware.

• Basic computer skills and EMR/EHR capability are required.

• Mandatory orientation and training specified by the government.

• Compliance with Agency Personal Identity Verification procedures and background checks (Level I and Level II).

• Contractor shall manage all patient information according to HIPAA, Privacy Act, and IHS policies.

• Adherence to IT system security policies and procedures is required.

• Completion of Information Systems Security Awareness (ISSA) training within 24 hours of start date.

• Contractor shall wear a government-issued identification badge.

• Government will provide facility-specific contractor identification badges (fee for lost/destroyed).

• Government will provide PPE, necessary equipment, supplies, and clinic space, except for uniforms, lab coats, and other personal medical instruments which are contractor-furnished.

• Government will provide facility-specific orientation/training if necessary.
