• Representation Section: Requires the offeror to check a box indicating whether they do or do not meet the definition of an "Indian Economic Enterprise" with "Indian Ownership."
• Required Fields: Name of Federally Recognized Tribal Entity, Name of the 51% owner of IEE Firm, Business Name, DUNS Number, Certifying Signature, and Print Name.

This form is critical for any bidder seeking to qualify for contracts or set-asides under the Buy Indian Act. Bidders must accurately complete this self-certification to demonstrate their eligibility. Failure to meet the criteria or provide accurate information can lead to disqualification or penalties.

Solicitation / RFP / RFQ

This solicitation is for the creation of a Storm Water Pollution Prevention Plan (SWPPP) for the Kayenta Health Center (KHC) located in Kayenta, Arizona. The project is a "turnkey" effort requiring the contractor to conduct a full site assessment and develop the SWPPP to contain potential diesel fuel spillage. This is necessary due to a past incident where diesel fuel spilled into the ground and a storm drainage pond, and no SWPPP was in place.

• Develop a SWPPP for diesel fuel storage and generator belly tanks.

• Conduct a full site assessment.

• Contractor must be knowledgeable of NFPA Codes, EPA cleanup standards, diesel fuel testing, and disposal of contaminated dirt.

• Contractor must furnish all tools, materials, and equipment.

• Vendor must have certifications to create SWPPP.

• The contractor shall prepare a proposal to complete this project.

• Project includes quoting mobilization and travel, documenting work, inspecting drainage from diesel fuel storage, and inspecting the pond.

• The contractor must notify KHC if EPA notification is needed.

• Period of Performance: July 1, 2026, to June 30, 2027.

• The contract is an Indian-Owned Small Business Economic Enterprise (ISBEE) set-aside.

• The solicitation issue date is June 2, 2026.

• The offer due date is June 17, 2026, at 1700 ET.

• Evaluation factors include Price, Technical Capability, and Timetable and Workplan.

• This acquisition is set aside exclusively for Indian-Owned Small Business Economic Enterprises (ISBEE).

• Attachment A contains the Statement of Work.
