This is a combined synopsis/solicitation for commercial products, issued as a Request for Quotation (RFQ) for Lab Testing Services. It is a 100% small business set-aside, utilizing simplified acquisition procedures for a firm-fixed-price purchase order. The contractor will provide all necessary personnel, equipment, tools, materials, supervision, and quality control for Laboratory Testing Services, as defined in Attachment 1 Performance Work Statement.

• Performance of Laboratory Testing Services.

• Contractor must provide all personnel, equipment, tools, materials, supervision, and quality control, except for Government Furnished items specified in Paragraph 3.0 of the PWS.

• The primary testing facility must be located within a 60-mile radius of Mansfield Lahm Regional Airport, Ohio 44903.

• Contract Type: Firm-Fixed Price (FFP).

• Place of Performance: 1947 Harrington Memorial Road, Mansfield, OH, 44903-8049.

• NAICS Code: 541380.

• Size Standard: $19 million.

• Quotes must be submitted electronically to james.kliewer.2@us.af.mil.

• Submission Deadline: 18 June 2026, 1:00 P.M. EST.

• Quote Organization: Three volumes required: Technical, Past Performance, and Price.

• Technical evaluation will be on an Acceptable/Unacceptable basis.

• Evaluation Factors: Technical, Past Performance, and Price.

• The Government will award to the responsible offeror whose quote is most advantageous, considering price and other factors.

• This solicitation is a 100% small business set-aside.

• No site visit is required.

• Costs incurred for quote preparation will not be reimbursed.

• Questions are due no later than 1:00 PM EST on 11 June 2026.

Statement of Work (SOW) / Performance Work Statement (PWS)

This Performance Work Statement (PWS) outlines the requirements for laboratory testing services to support the Mansfield Lahm Air National Guard Base.

• Provide all necessary personnel, equipment, tools, materials, supervision, and quality control for laboratory testing services.

• Conduct stormwater testing at four outfalls on base for reporting to the Ohio EPA, in accordance with the installation's individual NPDES Permit.

• Perform annual hazardous waste profile testing as directed by the Installation Hazardous Waste Management Plan.

• Period of Performance: 1 Base Year of 12 months, with three (3) 12-month option years.

• Place of Performance: Contractor's test facility must be located within 60 miles of the 179th Airlift Wing, Mansfield, Ohio.

• Services to be provided Monday through Friday, 0900 - 1700, excluding US holidays.

• Quality Control (QC): Contractor must develop and maintain an effective QC Plan (QCP) to ensure services meet PWS requirements. A final QCP is due 10 days after contract award.

• Quality Assurance (QA): Government will evaluate performance using a Quality Assurance Surveillance Plan (QASP).

• Reporting: Test results must be delivered electronically within 15 business days of sample receipt, in the form of a Certificate of Analysis.

• Contractor employees must comply with installation regulations, including fire, traffic, safety, and security.

• All employees must possess required licenses and be fully trained, licensed, certified, and qualified.

• Contractor must be recognized as a qualified testing agency by the State of Ohio.

• Employees requiring access to Army installations must complete AT Level I Awareness training.

• Contractor must comply with personal identity verification requirements.

• Zero tolerance policy for trafficking in persons.

• Organizational Conflicts of Interest (OCI) must be managed and reported.

• Government Furnished Property, Material, Equipment, and Services (GFP/M/E/S): NONE.

• Contractor Furnished Property, Materials, and Equipment (CFP/M/E): Contractor shall furnish everything required except as specified as Government-Furnished.

• Applicable Publications: Federal Acquisition Regulation, Defense Federal Acquisition Regulation Supplement, 40 CFR 260.11, and the Installation's NPDES Permit.
