Performance Work Statement (PWS)

To perform mold remediation and reconstruction at the USDA Forest Service Lake City Airtanker Base Administrative Operations building in Lake City, FL. The goal is to make the building fully operational and safely re-occupiable.

This project is divided into two phases:

• Phase 1 – Mold Remediation and Selective Demolition: Includes complete mold remediation, removal and disposal of mold-impacted materials (e.g., framing, insulation, flooring, cabinetry), HVAC system cleaning, facility-wide cleaning, and post-remediation clearance testing.

• Phase 2 – Reconstruction: Reconstructs remediated areas to pre-loss condition or better, including framing, sheathing, vapor barrier, insulation, flooring (LVP), cabinetry, millwork, and MEP systems.

• All mold remediation and demolition must comply with IICRC S520, NADCA ACR-2013, EPA 402-K-01-001, OSHA, and Florida state requirements.

• Post-remediation clearance testing must be passed, verified by a licensed mold assessor, before reconstruction begins.

• Re-remediation is required at no cost if initial clearance fails.

• Reconstruction materials must be new, commercial-grade, durable, and low-VOC.

• Moisture-resistant wall assemblies and commercial-grade LVT flooring are specified.

• Cabinetry and countertops must comply with ADA requirements.

• Appliances must be new, commercial stainless steel, and Energy Star certified.

• Final cleaning must render the building move-in ready.

• Location: Osceola National Forest, Lake City Airtanker Base, 475 SE County RD 245, Lake City, FL 32025.

• Period of Performance: Base period of 60 days from the Notice to Proceed (NTP). Work must begin no later than 10 days after NTP.

• Licenses & Certifications: Contractor must hold a valid Florida Mold Remediator license and required specialist certifications. Minimum 10 years of experience in commercial mold remediation and construction is required.

• Quality Control: Contractor is responsible for all materials, labor, and ensuring work meets industry standards and applicable codes (IBC, IEBC, IFC, IFGC, IGCC, IPC, IMC, IWUIC, NFPA 70, NFPA 101, FBC, ABAAS, ADA, ASHRAE, ISC, NFPA, OSHA, EPA).

• Safety: Contractor must comply with all safety regulations and provide a site-specific safety plan.

• Site Access: Subject to TSA badge policy and airport security requirements. Personnel may require escort within airfield security fences. The building will remain unoccupied during construction.

• Reporting: Daily reports and photos are required. Government Points of Contact (POCs) are listed for technical direction and access coordination.
