This is a combined synopsis and solicitation (RFQ HT941026Q2009) for commercial laundry services for the Naval Medical Center Nutrition Management Department. The contractor will provide annual laundry services for various items including Chef Smocks, Chef Pants, Aprons, Chef Hats, Table Clothes, and Cloth Napkins. Detailed requirements are specified in the Statement of Work (SOW), Attachment 1.

• Evaluation Factors: Award will be made on a "Past Performance Price Tradeoff" basis. Factors are:

  1. Technical Capability (Acceptable/Unacceptable)
  2. Past Performance (rated on confidence assessment)
  3. Price (evaluated for reasonableness)

• Technical Capability is a go/no-go factor. Past Performance and Price will be traded off to achieve best value.

• Offerors must be registered in SAM and have no active exclusions.

• The Service Contract Act is applicable. Wage Determination information is provided.

• Bidders must adhere to DBIDS access requirements for Naval installations.

• Numerous FAR and DFARS provisions and clauses are incorporated by reference. Key provisions include FAR 52.212-1 (Instructions to Offerors) and FAR 52.212-2 (Evaluation).

This document is a Past Performance Questionnaire (PPQ) used by the Defense Health Agency (DHA) Western Markets Contracting Division to gather feedback on an Offeror's past performance for a U.S. Government contract. The completed questionnaire is intended to be submitted with the Offeror's quote.

• Offeror Information: Fields for the name and address of the Offeror being evaluated.
• Project Information: Details about the contract or project being referenced, including contract number, title, location, award/completion dates, contract value, and a description of the work performed.
• Evaluator Information: Fields for the name, agency, address, phone number, email, and position of the person completing the questionnaire.
• Rating Scale: A standardized rating system from 'Exceptional' (E) to 'Unacceptable' (U), with 'Not Applicable' (NA) also available. Specific instructions are provided for using these ratings, particularly for 'Marginal' or 'Unacceptable' evaluations.
• Performance Areas: A series of 19 questions covering various aspects of past performance, including client relationships, subcontractor management, staffing, corporate management, quality of work, schedule adherence, cost control, compliance (labor, safety), documentation timeliness, and any issuance of negative notices.

Statement of Work (SOW) / Performance Work Statement (PWS)

This Statement of Work (SOW) outlines the requirements for laundry services to be performed for the Nutrition Management Department at the Naval Medical Center San Diego, CA.

• Pick up soiled linen and perform laundry services, including washing, sorting, folding, wrapping in plastic, and delivery.

• Services include cleaning and delivery of government-owned linens, clothing, and equipage.

• Specific items to be laundered include chef smocks, chef pants, aprons, tablecloths, cloth napkins, cleaning rags, and extended cold weather clothing.

• Special handling is required for contaminated laundry from health care facilities, including specific washing and drying procedures to prevent microbial contamination.

• Laundered items must be clean, dry, and free of lint, removable stains, and objectionable odors.

• Shrinkage, fading, and loss of strength must be avoided.

• Contaminated laundry must meet specific bacteriological count standards (not exceeding 10 colonies per RODAC plate with absence of staphylococcus aureus or coliform organism).

• Items must be packaged to protect from contamination.

• The period of performance is a base year from 09 MAR 2026 to 08 MAR 2027, with four (4) option years extending to 08 MAR 2031.

• Services are to be performed at the Naval Medical Center San Diego, CA.

• Pick-up of soiled linen is required every first Tuesday of the month (0700-1000).

• Drop-off of cleaned linen is required every second Thursday of the month (0700-1000).

• Service hours are Monday through Friday, 0700-1500, excluding Federal Holidays.

• Contractor facility must be capable of laundering normally soiled and contaminated linens and must adhere to sanitation standards and infection control precautions.

• Compliance with California State Regulations (Title 16, Division 6, State Board of Fabric Care, Articles 1-7) is required.

• A "keep separate" or "non-commingling" method must be used for pickup, sorting, laundering, folding, pressing, bundling, packaging, and delivery to ensure items are returned to the correct customer activity.

• Contractor personnel must be able to speak and understand English.

• The contractor must provide documentation of licenses/permits and a sample delivery ticket/receipt form upon request.
