This document is the "Indian Health Service Buy Indian Act Indian Economic Enterprise Representation Form." It serves as a self-certification for offerors to confirm they meet the definition of an "Indian Economic Enterprise" as required by the Buy Indian Act. This form is to be submitted in response to a Sources Sought Notice, Request for Information (RFI), or Solicitation.

• Self-Certification: Offerors must certify whether they meet the definition of an "Indian Economic Enterprise" with "Indian Ownership."
• Eligibility Period: The enterprise must meet the definition throughout the offer period, contract award, and the full contract performance period.
• Notification Requirement: Contractors must immediately notify the Contracting Officer if they no longer meet eligibility requirements.
• Documentation: Contracting Officers may require documentation of eligibility.
• Registration: Successful offerors must be registered with the System of Award Management (SAM).
• Legal Consequences: False or misleading information is a violation of law and punishable under 18 U.S.C. 1001, with false claims subject to penalties under 31 U.S.C. 3729-3731 and 18 U.S.C. 287.
• Required Fields: The form requires the Offeror to provide:
  • Name of Federally Recognized Tribal Entity (if applicable)
  • Name of the 51% owner of IEE Firm
  • Business Name
  • DUNS Number
  • Certifying Signature
  • Print Name

This form is critical for any bidder seeking to participate in opportunities designated for or influenced by the Buy Indian Act. Bidders must accurately complete this form to self-certify their eligibility as an "Indian Economic Enterprise." Failure to meet the criteria or accurately represent the business's status can lead to disqualification or penalties. It also highlights the need for SAM registration and potential documentation requirements.

Statement of Work (SOW) / Performance Work Statement (PWS)

To provide uninterrupted off-site laundry services for the Wagner Health Care Center (WHCC), ensuring compatibility with the facility's operating capacity on the Yankton Sioux Reservation.

• Furnish laundry services in compliance with specified requirements.

• Maintain laundry policies and procedures, posted visibly.

• Ensure physical separation between soiled and clean linen areas.

• Provide adequate hand washing facilities and clean, well-maintained toilet rooms.

• Maintain receiving/sorting areas under negative pressure to prevent contaminant migration.

• Finish/iron all linen items to recognized industry standards or specified requirements.

• Return clean linen in enclosed carts or clear plastic, grouped by type, to prevent contamination.

• Process linen to ensure sanitization without staining or damage, using specific wash cycles and temperatures (exceeding 160 degrees F for 25 minutes).

• Deliver clean linen sorted by type, with OR scrub suits in separate carts/packs.

• Provide a billing sheet with each delivery detailing total pounds of cleaned linen.

• Return irreparably damaged/permanently stained linen to the vendor.

• Physician/lab coats to be returned pressed.

• Linen processing temperatures to exceed 160 degrees F for 25 minutes.

• Wash and rinse cycle temperatures monitored accurately (+/- 3 degrees F).

• Clean linen tested for pH weekly; loads outside the accepted range re-laundered.

• Turnaround time for soiled linen pick-up and clean linen delivery not to exceed 48 hours.

• Clean linen not to remain on contractor's premises for more than 24 hours.

• Specific cleaning frequencies for various areas and equipment using approved detergent-germicides.

• Period of Performance: April 1, 2026, to March 31, 2027.

• Place of Performance: Wagner IHS Healthcare Center, 111 Washington Ave. NW, Wagner, South Dakota 57380.

• Facility requirements include non-absorbent, easily cleanable surfaces, proper ventilation, and adequate illumination levels.

• Personnel handling linen must adhere to strict hygiene practices, including clean clothing, hand washing, and separation of duties.

• Scales used for billing must be certified annually.

• Contractor must have a prior successful history providing laundry services to medical facilities.

• Invoices must be submitted electronically via the Invoice Processing Platform (IPP).

• Collections and deliveries scheduled for Monday, Wednesday, and Friday.
