This document is a combined synopsis/solicitation for commercial items, specifically laundry services for the National Hansen's Disease Program (NHDP). • The contractor shall provide non-personal laundry services.

• Past Performance: Requires three (3) past performance references from the last three years, similar in scope and complexity. CPARS ratings or a Past Performance Questionnaire (Attachment C) are required.

• Business Quote: Must propose labor categories, hours, unit pricing, discounts, and any other relevant pricing information. A completed and signed Standard Form 1449 is required.

• Award Type: Firm-Fixed Price.

• Period of Performance: April 26, 2026, to April 25, 2031.

• NAICS Code: 812332 (Industrial Launderers).

• Size Standard: $47.5M.

• PSC Code: S209 (Housekeeping Laundry/Dry-cleaning).

• Offerors must submit quotes in three (3) volumes: Technical, Past Performance, and Business.

• Quotes must be valid for at least 180 days from the receipt date.

• Submission Deadline: April 8, 2026, 12:00 PM ET.

• Evaluation Method: Lowest Price Technically Acceptable (LPTA).

• Award Criteria: Most advantageous to the Government, considering price and other factors.

• This acquisition is unrestricted, with all qualified offerors eligible.

This document is Attachment C, a Past Performance Questionnaire Letter and associated survey forms. It is provided to clients of firms responding to HRSA Solicitation No. RFQ 75R60126Q00001, "NHDP Laundry Services." The government is emphasizing past performance as a source selection factor and requires client feedback.

• Past Performance Questionnaire Letter: Informs the client about the solicitation and requests their completion of the enclosed questionnaire.
• Past Performance Information Survey Questionnaire: Collects information from the client regarding the company's performance, including:
  • Name of Company being reviewed
  • Client's Name, Agency, and contact information
  • Role in the contract (Project Officer, Contracting Officer, Contract Specialist)
  • Approximate percentage of work completed by subcontractors
  • General description or title of the contract
• Standard Contractor Performance Report: A detailed report for the client to complete, covering:
  • Contract details (Type, Number, Contractor Name, Award/Expiration Dates, Value, Supplies/Services)
  • Ratings for Quality of Product or Service, Cost Control, Timeliness of Performance, and Business Relations, with detailed definitions for each rating level (Unsatisfactory to Outstanding).

  • Admin Info for Project Officer/COR and Contracting Officer.
• Rating Guidelines: Provides detailed descriptions for each performance rating category (Quality, Cost Control, Timeliness, Business Relations) to ensure consistent evaluation.

Bidders must ensure their clients receive and complete this questionnaire accurately. The information provided by clients will be used as a significant evaluation factor for contract award. The deadline for returning the questionnaire to HRSA is April 8, 2026, at 12:00 PM EST.

This document, "Attachment D - Invoice Instructions," outlines the mandatory requirements for submitting invoices for payment under the contract. It details the use of the U.S. Department of Treasury's Invoice Processing Platform (IPP) and specific invoicing procedures for the Health Resources and Services Administration (HRSA).

• Mandatory Use of IPP: All invoice submissions for goods and services must be made electronically through the IPP system.
• Registration: New contractors will be enrolled by the Department of Treasury and must follow email instructions to register their Collector Account. Existing IPP users do not need to re-register.
• Invoice Submission Requirements: Invoices must comply with the applicable Prompt Payment clause or FAR 52.212-4.
• Additional HRSA Invoice Requirements:
  • Invoices must be submitted monthly or as per the payment schedule.
  • Separate invoices are required for each order under indefinite delivery vehicles.
  • Invoices must break out price/cost by Contract Line Item Number (CLIN).
  • Unique Entity Identifier (UEI) of the Contractor is required.
  • For Time and Materials or Labor Hour CLINs, supporting documentation for labor hours and material costs is needed.
  • For cost-reimbursement CLINs, monthly expenditures and cumulative amounts must be shown, including details on Direct Labor, Indirect Costs, Consultants, Travel, Subcontractors, Other Direct Costs, and Fee.

This document is critical for bidders to understand the payment process and invoicing requirements. Compliance with these instructions is necessary for timely payment upon contract award. Bidders should ensure their accounting systems and processes can meet these electronic submission and detailed reporting requirements.

This document is Attachment B, a Contractor Non-Disclosure Agreement (NDA) for Information Technology Systems Security Contractors. It outlines the terms and conditions under which contractors will access and protect sensitive information related to their official duties with the Department of Health and Human Services (HHS).

• Contractors must agree not to release, publish, or disclose sensitive information (including PII, non-public, confidential, and CUI) to unauthorized personnel.
• Contractors must protect such information in accordance with relevant laws and regulations.
• Contractors affirm they have received a briefing on their responsibilities and understand that violations may lead to criminal and civil penalties.
• The agreement requires signatures from the contractor, a witness, and specifies retention by the HRSA Contracting Officer Representative, Contractor's Contract Management, and the Individual Signatory.

This document is a mandatory requirement for contractors involved in IT systems security. Statement of Work (SOW)

To provide non-personal Laundry Services for the National Hansen's Disease Program (NHDP) in Baton Rouge, Louisiana.

• Laundry Pick-up and Drop-off: Contractor to pick up laundry on Tuesdays and drop off on Thursdays, with 7 days' notice from the COR. Pick-up/drop-off cycles not to exceed 24 in a 12-month period.

• Supplies and Equipment: Contractor to provide all necessary supplies and equipment for laundry services.

• Workload Requirements: Wash, dry, fold, and separate linens, towels, quilts, bath mats, and shower curtains for dormitory and clinical spaces. Estimated workload: 100 items or 60 pounds of linen per week.

• Government Furnished: Government will provide linens, towels, bedspreads, quilts, pillows, and infectious waste bags. Government will also supply storage space.

• Period: Base period of 12 months plus four 12-month option periods. Estimated start date: April 26, 2026.

• Place: 9181 Interline Avenue, Baton Rouge, Louisiana.

• Core Days and Hours: Tuesday and Thursday, 8:00 am - 4:30 pm.

• Federal Holidays: No services on Saturdays, Sundays, Federal Holidays, or other government closures. Duties resume the next business day if a holiday falls on a core day.

• Training: Key personnel must complete OSHA Standards training and maintain knowledge of CDC regulations for communicable diseases. HRSA Records Management Training is required for employees with access to Federal information or PII.

• Security: Strict adherence to Federal Records Act, NARA regulations, and HHS/HRSA information security and privacy policies is required. This includes safeguarding information, protecting confidentiality, integrity, and availability, reporting incidents within one hour, and complying with encryption standards (FIPS 140 validation).

• Section 508 Compliance: All products and services delivered must comply with Section 508 standards for accessibility, particularly for Information and Communications Technology (ICT).
