This document provides historical demand data for lenses, broken down by fiscal year (FY) and lens material/type. It includes both actual historical data and forecasted estimates for a 5-year ordering period (FY25-FY29). This data is intended to help potential bidders understand the historical usage and projected demand for various types of lenses.

• Lens Types: Includes various lens materials and designs such as CR-39, Poly, PL 1.60, Crown Glass, PL 1.56, PL 1.67, PL 1.595, PHTCHRM 1.50, PL 1.74, Trivex, PL 1.555, PL 1.549, and PL 1.740.
• Demand Data: Quantities are provided for different lens types (SV, Bifocal, Trifocal, PAL) and aggregated into 'Total' and 'Annual Total' for each fiscal year.
• Time Periods: Data covers actual historical demand from FY15 through FY24, a 5-year average of actual historical demand (FY20-FY24), and forecasted demand for FY25 through FY29.

This historical demand data is crucial for bidders to estimate the potential volume of lens requirements over the contract period. It allows for a data-driven approach to understanding the scale of the opportunity and preparing accurate pricing and resource allocation strategies. The forecasted demand provides insight into future needs.

Statement of Work (SOW) / Performance Work Statement (PWS)

The Department of Veterans Affairs (VA), Veterans Integrated Service Network 20 Optical Fabrication Lab (OFL), intends to solicit proposals for multiple Blanket Purchase Agreements (BPAs) to furnish ophthalmic lenses to eligible veterans. An estimated 137,215 lenses are required annually.

The vendor shall provide all materials, labor, equipment, and transportation for ophthalmic lenses. Lenses must meet specific American National Standards (ANSI Z80.1, Z80.3, Z80.9, Z87.1) and FDA regulations. Key requirements include:

• Lens Types: Single Vision (Finished & Semi-Finished), Bi-focal (FT-28, FT-35, FT-45 Semi-Finished), Tri-focal (7x28, 8x35 Semi-Finished), Progressive Adaptive Lenses (PAL) Semi-Finished.

• Materials: CR-39 plastic, 1.55 Mid-Index, 1.67 Hi-index, 1.74 Hi-Index, Polycarbonate, and Trivex.

• Features: Available in clear, photo-chromatic, and polarized options (all available colors). All lenses shall be coated unless no coated option exists. Lens blanks available in 65mm, 70mm, 75mm, and 80mm. Specialty lenses (lenticular, aspheric) are required.

• Specifications: Detailed power ranges and base curve requirements are specified for each lens type.

• Availability: Vendors must maintain 95% availability of awarded lenses.

• Warranty: Minimum one-year warranty against manufacturer material defects, coatings, and embedded defects.

• Lens Availability: 95% of awarded lenses must be available.

• Delivery Timeline: Ordered lenses must be provided within two (2) business days of order receipt.

• Backorder Notification: Contact OFL within 24 hours of identifying a backorder, providing an estimated time of arrival.

• Recall/Modification Notification: Notify OFL immediately of product recalls, removals, adjustments, or modifications.

• Stock Depletion Notification: Notify OFL within 24 hours of stock depletion for a specific lens.

• Period: One Base Year plus up to 4 Option Years.

• Place: Lenses delivered to the OFL at the Boise VA Medical Center, 500 W. Fort St. BLDG 28, Boise, ID 83702.

• Invoicing: Electronic submission via Tungsten Network or X12 EDI format is required. Itemized invoices must accompany each order.

• Ordering: No batching of orders; multiple orders may be submitted daily. Vendors must respond to telephonic, email, or electronic orders within 24 hours with order status.

• Shipping: Shipping charges are included in per-unit prices. Deliveries via 2nd day mail or comparable means.

• Government Purchase Card: May be used for initial payments; vendor must be Level II billing capable.

• Business Hours: OFL operates 6:00 a.m. – 4:30 p.m. M-F, Mountain Time.

• Federal Holidays: No services provided on listed Federal Holidays.
