This document is the "Indian Health Service Buy Indian Act Indian Economic Enterprise Representation Form." It serves as a self-certification for offerors to confirm they meet the definition of an "Indian Economic Enterprise" as required by the Buy Indian Act. This form is to be submitted in response to a Sources Sought Notice, Request for Information (RFI), or Solicitation.

• Eligibility Criteria: The enterprise must meet the definition of "Indian Economic Enterprise" at the time of offer, contract award, and throughout the contract performance period.
• Notification Requirement: Contractors must immediately notify the Contracting Officer if they no longer meet eligibility requirements.
• Documentation: Contracting Officers may require documentation of eligibility.
• Registration: Successful Offerors must be registered with the System of Award Management (SAM).
• Legal Consequences: False or misleading information is a violation of law and punishable under 18 U.S.C. 1001. False claims are subject to penalties under 31 U.S.C. 3729-3731 and 18 U.S.C. 287.
• Representation Section: Offerors must indicate whether they meet the definition of an "Indian Economic Enterprise" with "Indian Ownership."
• Required Fields: Name of Federally Recognized Tribal Entity, Name of the 51% owner of IEE Firm, Business Name, Certifying Signature, UEI Number, and Print Name.

This form is critical for any bidder seeking to participate in opportunities designated under the Buy Indian Act. Bidders must accurately complete this self-certification to demonstrate their eligibility. Failure to meet the criteria or provide accurate information can lead to disqualification or legal penalties. It also highlights the need for SAM registration.

Statement of Work (SOW) / Performance Work Statement (PWS)

To provide contract linen and floor mat laundry services for the Clinton Service Unit (CSU), which includes the Clinton Indian Health Center (CIHC), El Reno Indian Health Center (EIHC), and Watonga Indian Health Center (WIHC). The services are required to support healthcare operations without interruption.

• Provide cleaning services for all soiled linens, including blankets, towels, wash cloths, sheets, gowns, mammography capes, mops, microfiber mops, and microfiber cleaning cloths.

• Ensure all leased linens are cleaned separately from other facilities' linens.

• Return all items in good condition, clean, pressed, and uniform manner, covered with plastic for cleanliness and sanitary integrity.

• Provide floor mat exchange services for each facility.

• All linens and floor mats must be free of tears, rips, or markings.

• Services must comply with The Joint Commission standard IC.01.02.01 and Healthcare Laundry Accreditation Council (HLAC) standards.

• Linens must be cleaned in accordance with Joint Commission standard IC.01.02.01 and HLAC standards.

• Contractor must maintain a manifest documenting receipt and return of linens, including date, facility, personnel, pick-up/delivery designation, variances, detergent used, water temperatures, and compliance with standards.

• Contractor must provide a current policy and procedure manual.

• Contractor must be accredited by HLAC or demonstrate ability to comply with HLAC standards.

• Period of Performance: 12 months from date of award, with two (2) one-year option periods.

  • Option Year 1: 06/01/2027 – 05/31/2028
  • Option Year 2: 06/01/2028 – 05/31/2029

• Locations: Clinton Indian Health Center, El Reno Indian Health Center, and Watonga Indian Health Center.

• Contractor personnel may be subject to PIV credentialing requirements.

• The facilities are tobacco/smoke-free environments.

• Contractor personnel must contact the government point of contact upon arrival and are responsible for the security of all organizational information.

• Contractor must provide a toll-free telephone number for service calls, answered Monday through Friday, 8:00 am to 4:30 pm.

• Deliveries cannot be made after 4:15 pm. Driver needs a valid ID.

• The contractor shall furnish all personnel, materials, supplies, and equipment.
