Eligibility & Submission Requirements
Only facilities-based incumbent and competitive local exchange carriers certified by the State of Kentucky are eligible to submit quotations. Submissions must be sent via email to the Contracting Officer and include:
- Unique Entity Identifier (UEI) registered in SAM.
- Proof of Kentucky certification.
- A completed Pricing Schedule (Excel attachment) with accurate pricing (two decimal places) for all circuits in SOW Appendix 10. Quotations must be on an "all or none" basis.
