Statement of Work (SOW) / Performance Work Statement (PWS)

To procure 456 mattresses with protective encasements for 15 Missile Alert Facilities (MAFs), including the delivery of new items and the pick-up and disposal of old mattresses.

• Purchase and delivery of 456 mattresses and 912 protective encasements to 15 MAFs.
• Delivery includes unloading to the on-site vehicle garage.
• Pick-up and disposal of old mattresses within 5 days of notification, occurring 24 hours after new mattress delivery.

Mattresses:

• Quantity 441: Standard Twin XL (38"x80"x10")
• Quantity 15: Custom Size (34"x77"x10")
• Construction: Individually wrapped steel coil support system, high-density foam border for edge support.
• Comfort: CertiPUR-US® certified foam layers, "Medium-Firm" feel.
• Design: "Euro Top" construction.
• Safety: Compliant with federal flammability standards (16 CFR Part 1632 & 1633).

Protective Encasements:

• Quantity: 912 (two per mattress).

• Standard: Certified waterproof, barrier against bed bugs, dust mites, allergens, mold, liquids.

• Performance: Certified to ASTM F1670 and ASTM F1671.

• Design: Zippered, fully enclosing on six sides with a bed bug prevention feature.

• Contractor is responsible for all delivery and pick-up logistics.

• Delivery coordination with Government POC 72 hours prior.

• Manufacturer's standard commercial warranty for mattresses.

• Delivery to 15 Missile Alert Facilities (locations detailed in Appendix A).

• Delivery within 30 days of shipping availability.

• Environmental compliance for disposal of old mattresses.

• Contractor personnel require base identification and vehicle passes; specific procedures and documentation (REAL ID, passport, birth certificate, SSN) are detailed.

• Adherence to security protocols, including background checks and potential FPCON measures.

• No electronic devices or weapons allowed on facility.

• Drivers must have valid licenses, registration, and insurance.

• All personnel and vehicles must be on the Entry Authorization List (EAL) at least two weeks prior to work commencement.

Solicitation / RFP / RFQ

This is a Request for Quotation (RFQ) for commercial items to procure 456 mattresses, 912 protective encasements, and the pick-up and disposal of all old mattresses from 15 Missile Alert Facilities (MAFs). The acquisition is set aside for 100% Small Business Concerns.

• Purchase of standard twin XL mattresses (38" x 80" x 10") and custom mattresses (34" x 77" x 10").

• Purchase of 912 protective encasements.

• Pick-up and disposal of old mattresses.

• Full performance, including delivery and pickup, must be completed no later than 90 days After Receipt of Order (ARO).

• Detailed product specifications and logistical requirements are provided in Attachment 1 – MAF Mattress SOW.

• Contract Type: Firm-Fixed Price (FFP).

• Place of Delivery/Performance/Acceptance/FOB Point: F.O.B. Destination, Minot AFB, ND 58705.

• Questions Deadline: Thursday, 9 April 2026, 8:00am CST.

• Quote Submission Deadline: Friday, 17 April 2026, 1:00pm CST via email.

• Quote Validity: Must be valid through 15 June 2026.

• Evaluation Criteria: Best Value determination based on Technical Capability (pass/fail), Price, and Past Performance. Technical capability is pass/fail. Past performance is approximately equal in importance to price.

• Submission Requirements: Quotes must include warranty terms, acknowledgment of the 90-day ARO delivery schedule, product specification sheets, and color photographs. Pricing must be submitted on Attachment 2 – MAF Mattress Price Sheet. Past performance information may be submitted using Attachment 3 – Past Performance Information Form (PPIF).

• The Government intends to make an award based on initial quotations without post-submittal inquiries.
