Statement of Work (SOW) / Performance Work Statement (PWS)

To procure 456 mattresses and 912 protective encasements for 15 Missile Alert Facilities (MAFs), including the pick-up and disposal of old mattresses.

• Purchase and delivery of 456 mattresses and 912 protective encasements to 15 MAFs.
• Delivery includes unloading to the on-site vehicle garage.
• Pick-up and disposal of old mattresses within 5 days of new mattress delivery.

Mattresses:

• Quantity 441: Standard Twin XL (38" W x 80" L x 10" H +/- 0.5")
• Quantity 15: Custom Size (34" W x 77" L x 10" H +/- 0.5")
• Construction: Individually wrapped steel coil support, high-density foam border, CertiPUR-US® certified foam comfort layers.
• Feel: Medium-Firm (supportive yet cushioned for spinal alignment).
• Design: Euro Top.
• Safety: Compliant with federal flammability standards (16 CFR Part 1632 & 1633).

Protective Encasements:

• Quantity: Two per mattress (912 total).

• Protection: Waterproof, barrier against bed bugs, dust mites, allergens, mold, liquids.

• Certification: Third-party tested to ASTM F1670 & F1671.

• Design: Zippered, fully enclosed (6 sides) with bed bug prevention feature.

• Delivery of new mattresses within 30 days of shipping availability.

• Pick-up of old mattresses within 5 days of availability.

• Performance hours: 0800-1700, excluding Federal Holidays.

• Locations: 15 Missile Alert Facilities (locations detailed in Appendix A, distances from Minot AFB in Appendix B).

• Delivery & Pick-up: Contractor coordinates with Government POC 72 hours prior. Old mattresses available for pick-up 24 hours after new mattress delivery.

• Warranty: Manufacturer's standard commercial warranty for mattresses.

• Environmental Compliance: Adherence to all applicable regulations for disposal of old mattresses.

• Government Furnished Property/Services: Water and sewage (if available), base/local telephone service. Contractor covers long distance calls.

• Security: Contractor personnel require base identification and vehicle passes. Specific procedures for obtaining access, including REAL ID or passport requirements, background checks, and submission of personal information (DOB, SSN) at least two weeks prior to work. No electronic devices or weapons allowed on facility. Drivers must have valid license, registration, and insurance. Vehicles must be on Entry Authorization List (EAL).

• Heightened Security: Contractor must still deliver/pick up unless notified otherwise, expecting potential delays and restrictions.

Questions & Answers / Clarifications

This document provides responses to vendor questions regarding the RFQ FA452826QRS01 for MAF Mattresses, clarifying requirements and logistics.

• Delivery Vehicles: Smaller vehicles should be anticipated for all 15 Missile Alert Facilities (MAFs) as standard commercial delivery vehicles, including 53-foot trailers, cannot access all locations. A standard 26-foot box truck can access the vehicle garages.
• Loading Docks & Unloading: No loading docks are available. Delivery is limited to unloading at the on-site vehicle garage. The contractor is solely responsible for off-loading.
• Mattress Requirements: Euro tops are not strictly required; a quilted tight-top with extra cushion is acceptable. Mattresses must meet fire compliance (16 CFR 1632/1633), requiring a spec sheet for proof. The "Medium-Firm" feel is defined by the SOW description, not a specific IFD rating. The corrected mattress and encasement dimensions are detailed in Attachment 1 – MAF Mattress SOW.
• Disposal: The contractor is responsible for all disposal logistics of old mattresses; no disposal on government property.
• Technical Submission: A product spec sheet is sufficient for the product-specific portion of the technical submission. Color photographs are requested but not mandatory. Comparable commercial delivery experience will be considered for past performance.
• Delivery & Pick-up: Phased delivery is allowed if within the SOW timeframe. Old mattresses must be picked up within 5 days of each respective delivery. The contractor has flexibility in planning the delivery route and order, but the plan must be communicated a week in advance.
• Period of Performance: The estimated EAL processing time is 30 days, and the period of performance has been increased to 120 days ARO to account for this. Delays due to MAF closures will be addressed under FAR Clause 52.212-4(f) for excusable delays.
• Limitations on Subcontracting: Clause 52.219-14 has been removed from the solicitation.
