Statement of Work (SOW) / Performance Work Statement (PWS)

This PWS defines the requirements for preventive maintenance (PM) and unscheduled maintenance (UM) for Material Handling Equipment (MHE) at commissary system locations operated by the Defense Commissary Agency (DeCA) within the contiguous United States, Alaska, Hawaii, and Puerto Rico.

• The Contractor shall perform PM and UM of MHE at specified commissary locations.

• PM is scheduled maintenance to ensure equipment operates functionally.

• UM is unscheduled maintenance or repair performed on an as-needed basis to ensure equipment remains operational.

• The contract will be Firm Fixed Price (FFP) for PM services and Time and Material (T&M) for UM repairs.

• MHE covered includes various types of electric and propane/gasoline forklifts, pallet jacks, and battery chargers.

• PM intervals are every three (3) months.

• UM repair estimates must be provided to the COR within 16 hours of a service call.

• UM repairs require COR approval; repairs under $2,500.00 are approved by COR, while those over $2,500.00 require COR and DeCA HQ EMT approval.

• UM repairs must be completed within 7 working days of approval.

• A warranty is required for replacement parts and workmanship.

• Contractor must maintain cleanliness of work areas and dispose of materials properly.

• The period of performance is based on market research and will be a Firm Fixed Price for PM and Time & Material for UM.

• Work is to be performed on-site at DeCA commissary locations.

• Contractor personnel must be certified by Automotive Service Excellence (ASE) and possess mechanical aptitude.

• Contractor must comply with OSHA, DOD, DeCA, and Installation safety regulations.

• Contractor is responsible for safeguarding government property.

• Contractor must provide a phone number and email address for service calls.

• Contractor must provide a service ticket for each PM and UM, signed by the COR, and maintain a service history report.
