Statement of Work (SOW) / Performance Work Statement (PWS)

To provide Preventive Maintenance (PM) and Corrective Maintenance (CM) services for Material Handling Equipment (MHE) at Defense Logistics Agency (DLA) Disposition Services (DS) locations in Jacksonville and Eglin, Florida. The goal is to ensure MHE is consistently operational to support DLA DS daily operations.

• Preventive Maintenance (PM): Perform at least one annual PM per MHE item, adhering to Original Equipment Manufacturer (OEM) manuals. This includes inspections, fluid top-offs, and replacement of normal wear parts. PM services are to be performed on-site, with advance notification to the COR. Specific inspections include fork inspections (per ANSI/ITSDF standards) and checks of electrical systems, batteries, and charging stations for electric MHE.

• Corrective Maintenance (CM): Assess, inspect, and troubleshoot reported deficiencies within five business days. Provide written estimates for repairs, including detailed parts, labor, and shipping costs. Repairs require OEM-recommended parts, no rebuilt parts. Upon approval, repairs must be completed within five business days of parts receipt.

• Equipment: Services cover electric-powered equipment, associated batteries, and charging stations. MHE lists are provided in Attachments 2 and 3 and are subject to change.

• PM services must be completed within ten business days of commencement unless otherwise coordinated.

• CM repairs must be completed within five business days of parts receipt.

• Contractor must provide weekly status updates on pending repairs, parts orders, and estimated completion dates.

• Contractor must maintain a Quality Control Plan (QCP) to ensure services meet PWS requirements.

• Period: One (1) 12-month base period with two (2) 12-month option periods, not to exceed three (3) years.

• Place: Services are to be performed on USG premises at DLA DS Jacksonville and Eglin facilities, during normal business hours, except on federal holidays or during facility closures.

• Contract Type: Firm Fixed Price (FFP) time and materials.

• Personnel: Contractor must provide qualified personnel, including Certified Forklift Technicians (CFTs).

• Access & Security: Contractor must coordinate base access, comply with all installation security policies, and ensure employees carry proper identification.

• Property: USG will provide workspace and utilities. Contractor must provide all other necessary property, materials, equipment (including PPE), and services.

• Spill Responsibility: Contractor is solely responsible for cleaning up spills caused by their actions, at no cost to the USG, in compliance with federal, state, and local regulations.

• Invoicing: Monthly invoicing through the DoD Wide Area Workflow (WAWF) system.

• Contract Minimum/Maximum: Minimum $2,500; Maximum 150% of the estimated contract value. No minimum for option periods.
