This document, "Attachment 3 Experience Information Sheet," is a form for offerors to provide relevant past experience information. It is designed to help the government evaluate the offeror's capability to perform the required services.

• Maximum of 3 relevant contracts will be evaluated. Submitting more than three will not result in additional evaluation.
• For each contract, offerors must provide:
  • Contract Number
  • Name of Contractor (Offeror or subcontractor)
  • Name, email, and phone number of an English-speaking Point of Contact (POC)
  • Location where the service was performed
  • Date of the period of performance
  • Detailed information of the service performed, including explanations of any performance problems, issues, or conflicts and their resolutions.
  • Specific details on how the offeror approached services related to: Food Operations, Food Facilities and Equipment, Water Portability, Cleaning and Sanitizing, Pest Control, Employees, Food Defense, and Food Handling.
• Offerors are encouraged to provide pictures or other descriptive information.

This attachment is crucial for bidders to demonstrate their relevant experience. Bidders must carefully select up to three of their most relevant contracts and provide comprehensive details as requested to support their proposal. Failure to provide complete and accurate information may impact the evaluation of their past experience.

Statement of Work (SOW) / Performance Work Statement (PWS)

To provide US Government and associated personnel with catering food services in the Philippines.

• Provide non-personal catering services, including all personnel, equipment, supplies, facilities, and transportation.

• Offer various meal styles: Bento Style, Buffet Style, and special occasion meals.

• Cater to diverse dietary needs including Western, Philippine, and vegetarian options.

• Services include preparation, serving, delivery, and distribution of meals at designated dining facilities.

• Maintain facilities in accordance with U.S. Department of Health and Human Services Food Code and local standards.

• Be flexible to changes in performance periods, quantities, and services with 24-hour notification.

• Adherence to quality standards and acceptable quality levels outlined in the PWS.

• Compliance with health inspection standards (e.g., GMP/HACCP certifications, ServSafe).

• Maintain proper food temperatures (cold foods at 41°F/5°C or lower, hot foods at 135°F or higher).

• Implement proper thawing procedures and storage/labeling.

• Ensure all food contact surfaces are cleaned and sanitized.

• Maintain a sanitary condition of facilities and equipment.

• Provide potable water from an approved source.

• Adhere to environmental compliance and hazardous material disposal regulations.

• Contractor personnel must be medically screened and trained in hygiene and food safety.

• Meals must align with U.S. Army nutritional guidance and "Go for Green 2.0" standards.

• Contract Period of Performance: June 2026 - June 2031 (12-month base plus four 12-month option years).

• Place of Performance: Various zones within the Philippines, including Central Luzon, Northern Luzon, Southern Luzon, Northern Islands, and areas outside of Luzon.

• Contractor must comply with the Philippines Ministry of Health Guidance.

• Provide personnel protection equipment (PPE) and enforce safety protocols.

• Submit a Quality Control Plan (QCP) and Food and Water Risk Assessments (FWRAs).

• Report issues affecting performance within 2 hours.

• Personnel must be fluent in English.

• Contractor is responsible for all security requirements, including installation access and badging.

• Contractor travel is at their own expense.

• Adhere to safety and accident prevention regulations (AR 385-10, DA PAM 385-10, OSHA CFR Title 29 Part 1910).

• Provide a Contract Manager and designated alternate available to meet with COR/Contracting Officer.

• Contractor employees must identify themselves as such to avoid misrepresentation.

• No verbal or physical abuse tolerated from contractor personnel.

This document is a pricing sheet (Excel file) that outlines the CLINs (Contract Line Item Numbers) and associated quantities for catering services across five different zones in the Philippines. It details the breakdown for both Bento Style and Buffet Style meals, as well as Special Occasion meals.

• CLINs: The document lists CLINs for five ordering periods (CLIN 0001 to CLIN 4001), each corresponding to a specific year of performance.
• Supplies/Services: Specifies "Philippines Catering Meals Bento Style and Incidentals," "Philippines Catering Meals Buffet Style and Incidentals," and "Philippines Catering Special Meal and Incidentals for Special Occasion."

Solicitation / RFP / RFQ

This is a Combined Synopsis/Solicitation (RFQ W91QVP-26-Q-A001) for commercial catering food services in the Philippines. The 413th Contracting Support Brigade (CSB), 921st Contracting Battalion intends to award multiple Firm Fixed Price Indefinite Delivery/Indefinite Quantity (IDIQ) contracts. The procurement is full and open competition.

• Catering food services for the Philippines.

• Vendors must submit a proposed one-month menu (max 5 pages, English).

• Vendors must submit Food and Water Risk Assessments (max 10 assessments, not exceeding 2 pages each, in English).

• Vendors must submit no more than three (3) relevant past experiences, each no more than one page, using Attachment 3.

• Contractors must be registered in SAM.gov and WAWF.

• Contract Type: Multiple Award IDIQ (Firm Fixed Price).

• Estimated Total Program Value: Approximately $4,987,117.96 (including option).

• Each awardee will have a total estimated contract value of approximately $1,484,376.18.

• Minimum Order Limit: $1,000.00.

• Maximum Order Limit: $4.9 Million.

• Minimum Guarantee per awarded contractor: $1,000.00.

• Period of Performance: Five years, with five 12-month ordering periods, plus a potential 6-month extension (FAR Clause 52.217-8).

• Ordering Periods:

  • Period 1: 22 Jun 2026 - 21 Jun 2027
  • Period 2: 22 Jun 2027 - 21 Jun 2028
  • Period 3: 22 Jun 2028 - 21 Jun 2029

• Submission Deadline: Quotes due by 0800 HST on 30 APR 2026.

• Submission Method: Via email to SSG Kim, Joo (joo.j.kim.mil@army.mil) and MSG Gutierrez, Gloria (gloria.i.gutierrez7.mil@army.mil).

• Questions Deadline: Questions must be submitted by 0800 HST on 27 APR 2026.

• Evaluation Criteria: Best value, comparative evaluation based on Price and Other Factors. Factors include:

  • Factor 1: Technical (Proposed Menu, FWRA, Relevant Past Experience)
  • Factor 2: Past Performance
  • Factor 3: Price

• Quotes must be valid for at least 90 days after submission.

• This procurement is full and open competition.

• Contractors must complete and sign the "Combined Synopsis and Solicitation" and the "Pricing Sheet" (Attachment 2).

• All prices must be in US Dollars (USD).

• Vendors must be actively registered in SAM.gov with an ACTIVE status.

• WAWF registration is required for payment processing.
