Statement of Work (SOW) / Performance Work Statement (PWS)

To provide service, calibration, and repair to all medical and dental equipment within the MNARNG (Maryland National Guard). This service was historically provided by USAMMA but is no longer directly supported for Compo 2 and Compo 3 organizations.

• Provide all necessary personnel, equipment, tools, materials, supervision, and quality control for Medical and Dental Maintenance Services.

• Services include calibration, servicing, and repair of medical and dental equipment.

• Performance will be evaluated based on a Quality Assurance Surveillance Plan (QASP).

• Key performance objectives include:

  • 100% of equipment calibrated per applicable manuals.
  • 100% of equipment serviced per applicable manuals.
  • 100% of repair work completed within specified timeframes (30 days from authorization).

• Reporting requirements include:

  • Annotation of all repair work on form DA 2404.
  • Submission of calibration and service records on form DA 2404 to the COR within 15 days of completion.
  • Notification to COR of repair work required within 15 days.
  • Provision of cost breakdown for repairs.
  • Provision of a list of unrepairable equipment within 15 days of identification.

• Period of Performance (PoP): One (1) Base Year of 12 months with four option years.

• Place of Performance: Services to be provided at the designated location listed within Technical Exhibit 3.

• Hours of Operation: Monday through Friday, 0800 - 1630, excluding recognized US holidays or Government facility closures.

• Personnel Qualifications: All employees must possess required certifications for operating, servicing, calibrating, and repairing equipment. The Government does not provide training; contractors must ensure personnel are fully trained, licensed, and certified.

• Security: Compliance with all applicable installation/facility access and local security policies and procedures is required. This includes background checks, personal identity verification, Awareness Training (AT) Level 1, and iWATCH training.

• Telework: Teleworking is not authorized, except in furtherance of Continuity of Operations Planning (COOP) during national emergencies.

• Quality Control: Contractor must develop and maintain an effective Quality Control Plan (QCP).

• Reporting: Service Contract Reporting is required annually via a secure data collection site.

• Travel: Contractor travel to locations identified in Technical Exhibit 3 is authorized, with expenses consistent with the Joint Travel Regulation (JTR), requiring COR recommendation and KO authorization.
