• Tax ID, UEI Number, Cage Code, NAICS, Company Web Page
• Small Business status and other socio-economic classifications (e.g., 8(a), HUBZone)
• SAM Registration status
• Indirect Rate Agreement or Forward Pricing Rate Agreement
• Cost accounting system compliance (DFARS 252.242-7006) or pre-award survey (SF 1408)

Product Information:

• Supply Support: Familiarity with DLA E-CAT, availability via E-CAT, identification of repair parts (name, part number, price, dimensions), repair kits, MTTR, MTBF, and time estimates for repairs/calibration/maintenance.
• Maintenance Planning and Management: Maintenance strategy, maintenance agreements, operator/maintainer telephonic support, OEM notification process for alerts/recalls/change orders, and Level of Repair Analysis (LORA).
• Packaging, Handling, Storage and Transportation: Storage conditions (temperature, humidity), packaging challenges for hardened cases, and labeling requirements (NSN, IUID).
• Technical Data: Copyrights license for manuals, and requirements for Service Manuals and Operator Manuals (content, format, MIL-PRF-32216B compliance).
• Support Equipment: Specialized tools for operation/maintenance.
• Training and Training Support: Operator/maintainer training plans, online sustainment training resources, and training videos.
• Facilities and Infrastructure: Power requirements (AC/DC, Volts, Amps, Watts, Hertz), battery type, charge time, size, and weight.
• Computer Resources and Design Interface: Operating system, application software, and DoD Risk Management Framework (RMF) / Authorization to Operate (ATO) status.

Bidders must complete this form accurately and thoroughly to provide the government with the necessary data to evaluate their company's capabilities and the supportability of their proposed products. Failure to provide complete information may impact the evaluation of their response.
