This document is a Business Management Questionnaire. It is designed to collect information from prospective contractors to assist the Government in determining responsibility in accordance with Federal Acquisition Regulation (FAR) Part 9.

• Past Government contracts performed that are similar to the solicitation requirements (up to five).
• Details for each contract include customer name and address, cognizant Contracting Officer, telephone number, contract type, contract number, amount, status, and percentage of facility and staff devoted to the contract.
• Any contracts awarded and subsequently defaulted by any Federal agency within the last 36 months.
• Bank reference, including bank name and address, point of contact, and telephone number.

Bidders must complete this questionnaire accurately and thoroughly. The information provided will be used by the Government to assess the bidder's capability and responsibility to perform the contract. Failure to provide complete and satisfactory information may impact a bidder's eligibility for award.

Solicitation / RFP / RFQ

This solicitation (FN1597-26) is for an Indefinite Delivery/Indefinite Quantity (ID/IQ) contract for insulation (fiberglass overlay) and mineral board. These materials are intended for use in the manufacture of office furniture. The government is procuring these items to meet its needs for office furniture production.

The solicitation details specific requirements for several items, including:

• Mineral Board (Items #1 & #15): Specific dimensions (7/16"x60"x78"), density, MOR, MOE, tensile strength, hardness, dimensional stability, thermal conductivity, R-Value, NRC, STC, flame spread, smoke developed, permeance, and weight.

Samples: Offerors who have not recently supplied similar products must submit two samples of the offered product.

ISO 9001 Certification: Facilities must be ISO 9001 certified, and proof must be submitted with the proposal.

• Period of Performance: Five years from the award date.

• Estimated Value: Guaranteed Minimum: $1,000.00. Maximum Not-to-Exceed Total Cost: $3,000,000.00.

• Set-Aside: Unrestricted.

• Offer Due Date: Monday, April 27, 2026, at 2:00 PM EST.

• Questions Deadline: Monday, April 13, 2026, at 2:00 PM EST.

• Submission Method: Offers must be submitted signed and dated via postal mail and email to peyton.perry@usdoj.gov.

• Evaluation: Offers will be evaluated based on price and other factors, with technical and past performance rated on a Pass/Fail basis. A tradeoff process may be used, allowing for award to other than the lowest-priced offeror.

• Proposal Contents: Must include SF 1449, Amendments, SAM Registration, Unique Entity ID, TIN, Business Management Questionnaire, Past Performance Surveys, and Pricing Excel Sheets.

• Set-Aside: This acquisition is unrestricted.

• SAM Registration: Required.

• Business Management Questionnaire: Required, with three references from the past three years.

• All solicitations and amendments are available on SAM.gov.

• Offerors must hold prices firm for 120 calendar days from the offer receipt date.

• UNICOR may add other factories or materials via modification.
