Scope of Work
The contractor shall provide licensed facilities, equipment, licensed funeral service practitioners, motor vehicles, professional services, and supplies for the care, preparation, and disposition of remains. Services must adhere to the detailed Performance Work Statement (PWS) (Attachment 1), which includes disinfection, preservation, restoration, embalming (for viewable and non-viewable remains), restorative art, dressing, casketing, and transportation. Cremation services are limited to subsequent portions or retained organs. Performance is required within a four-hour radius of Keesler AFB, generally within 24 hours of notification. A Quality Control Plan (QCP) is required.

This is a Firm Fixed-Price Blanket Purchase Agreement (BPA) with an ordering period from June 1, 2026, to May 31, 2031 (five years). Submission & Evaluation
Offerors must submit quotes electronically in three volumes: Price (Attachment 2), Technical (Attachment 3), and Site Inspection. Required submissions include Offeror Name, Physical Address, Point of Contact, SAM Registration/DUNS/Cage Number, Small Business Identification, completed Attachments 2 and 3, and recent inspection documentation (within 12 months). Failure to provide recent inspection documentation will necessitate a pre-award site inspection. Evaluation will be based on Price, Technical, and Site Inspection, with Technical and Site Inspection weighted equally and approximately equal to cost or price. An "Acceptable" rating is required in all evaluation parts for award.
