• Information Required: Includes Sponsor Information (name, employee ID, contact details, business unit, program, contract), Foreign Visitor Information (name, company, address, citizenship, etc.), Visit Details (dates, classified visit status, discussion of controlled unclassified information), Serco facility, and Purpose of Visit.
• Additional Sections: Includes fields for Security Comments and Global Trade Compliance Comments.
