Submissions, in PDF format, must be sent electronically to Clarion Miller (pete.miller@nih.gov) with a courtesy copy to Matthew Heavey (matthew.heavey@nih.gov), referencing "Handyman BPA" in the subject line. Required submission elements include:
- UEI#, Tax ID#, Point of Contact, Company website, Business size/socioeconomic category.
- Capability Statement covering Relevance of Experience, Volume and Consistency, Quality and Timeliness, Response Capability, Overall Performance, and client POCs.
- Bonding letter.
- Completed pricing table (Attachment 1) detailing labor/material mark-up/overhead/profit rates.
- Acknowledgment of GPC acceptance up to $15,000.
- Completed Past Performance Questionnaire (PPQ) (Attachment 4), which bidders must forward to clients for direct submission to NIH.
