Special Requirements for Physicians

Other requirements include a personal or Federal DEA number, current Basic Life Support (BLS) certification, a fitness for duty certificate, and no history of specific adverse actions. Immunizations, English proficiency, basic IT knowledge, and successful completion of Level I and Level II background checks are also mandatory. This is a non-personal services contract, requiring the contractor to act as an independent contractor and maintain liability insurance.
