Quick Summary

The National Institute of Health, Clinical Center (NIH-CC) has issued a Notice of Intent to Sole Source for the renewal, maintenance, and consulting services related to the proprietary NoteCetera and ICDx applications. The NIH-CC intends to negotiate with Working Diagnosis LLC, the sole vendor for these critical clinical documentation tools. Capability statements from other interested parties are due by March 5, 2026.

Purpose

This acquisition is for the continued support of the ICDx and NoteCetera applications, which are custom-engineered and deeply integrated into the NIH CRIS electronic health record. These tools are essential for structured problem list maintenance and efficient clinical note generation, directly impacting clinical documentation workflows at the NIH Clinical Center.

Scope of Work

The requirement includes:

  • Annual Maintenance: Licensing, upgrades, and technical support for both ICDx and NoteCetera.
  • Consulting Services: 100 hours of standard consulting, with an additional 300 hours available as part of Option A. These services ensure continued compatibility, software updates, bug fixes, Tier 2 technical support, and expert guidance for application enhancements and workflow improvements.

Contract & Timeline

  • Type: Notice of Intent to Sole Source (Presolicitation)
  • Intended Contractor: Working Diagnosis LLC
  • Period of Performance: February 25, 2026 – February 24, 2027 (Base Year), followed by an option year from February 25, 2027 – February 24, 2028.
  • Place of Performance: NIH Clinical Center in Bethesda, MD
  • Set-Aside: None (Sole Source)
  • Response Due: March 5, 2026, by 4:00 PM EST
  • Published: March 3, 2026

Justification

The sole source justification is based on the proprietary nature of ICDx and NoteCetera, which were custom-engineered for NIH and are deeply integrated into the CRIS system. Working Diagnosis LLC is the only vendor possessing the proprietary code, technical expertise, and integration knowledge required to maintain and enhance these tools, as per FAR 13.106-1.

Action Items

This is not a request for proposal. However, responsible sources who believe they can provide the required services may submit a capability statement via email to lu-chang.lu@nih.gov by the due date. The subject line must reference NOI-CC-26-001724. The Government will consider all responses, but a determination not to compete remains at its discretion.
