• Representation Section: Bidders must indicate whether they represent that their offer meets the definition of an "Indian Economic Enterprise" with "Indian Ownership."
• Required Fields: Name of Federally Recognized Tribal Entity, Name of the 51% owner of IEE Firm, Business Name, DUNS Number, Certifying Signature, and Print Name.

This form is critical for any bidder seeking to qualify for contracts under the Buy Indian Act. Bidders must accurately complete this self-certification to demonstrate their eligibility. Failure to meet the IEE definition or provide accurate information can lead to disqualification or penalties. Notice of Intent to Award

This notice indicates the Navajo Area Indian Health Service's intent to award multiple Blanket Purchase Agreements (BPAs) to Red Heritage Medical, LLC., and Bighorn Ridgerunners, LLC. These BPAs will cover Quality Control, Reagents, and Consumable Supplies for testing in chemistry, urinalysis, microbiology, and hematology on an as-needed basis for patient testing at the Dzilth-Na-O-Dith-Hle Health Center in Bloomfield, NM.

• The anticipated period of performance is a base period of one year from the date of award, with four option years.

• The North American Industry Classification System (NAICS) code is 325413 – In-Vitro Diagnostic Substance Manufacturing.

• The small business size standard is 1,250 employees.

• This notice is not a request for quotations or a solicitation.

• Other sources believing they can fulfill the government requirements are encouraged to submit their capability statement, price lists, and catalogs via email to the point of contact.

• The deadline for submitting capability statements is 03/07/2026 at 04:30 PM MT.

• The government may contact capable vendors to establish additional BPAs if deemed to be in the best interest of the government.

• No further awards will be made unless the need for these BPAs changes.

• The establishment of multiple BPAs is intended to constitute maximum practicable competition.
