Purpose & Scope
NCO 16 intends to procure the AHCP software to provide health literacy appropriate patient discharge instructions for the GVSM VAMC in Jackson, MS. The required software solution must include:
- Creation of health literacy appropriate patient discharge instructions.
- Report functionality for quality improvement and performance tracking.
- Administrative interface for user management, configuration, and auditing.
- Integration with the hospital-based VistA software for evidence-based discharge instructions.
- Customization to match GVSM VAMC branding, including logo placement.
- Addition of specific Congestive Heart Failure (CHF) discharge instructions.
- Creation and printing of a Care Plan booklet for every discharged veteran, detailing Site Logo, Patient Aligned Care Team, Demographics, Medications, Appointment Information, care instructions, diet/activity, and My HealtheVet information.
- Installation, configuration, and customization of the AHCP software on designated servers.
- Comprehensive training for all staff members using the software.

Submission Requirements
This notice is NOT a request for quotations. It is an Intent to Sole Source.
- Interested parties capable of providing the required AHCP software may express interest by submitting a capabilities statement via email to kolbi.barton@va.gov.
- The deadline for submissions is June 16, 2026, at 10:00 AM CST.
- Capabilities statements must provide clear and unambiguous evidence of capability.
- All interested parties must be registered with SAM.gov.
- The Government will determine, at its sole discretion, whether to compete the requirement based on viable responses received. If no viable responses are received, NCO 16 will proceed with negotiations solely with Engineered Care, INC.
