Statement of Work (SOW) / Performance Work Statement (PWS)

To provide annual preventive maintenance, calibration of sensors, and one year of parts, labor, and onsite service for the REES Environmental Monitoring System at Yokota Air Base, Japan, for the 374th Medical Group.

• Perform annual preventive maintenance (PMI) and calibration of the REES Environmental Monitoring System.

• Include all travel, labor, as-needed parts, materials, and manufacturer's authorized field modification instructions.

• Provide all software and hardware updates, including field modifications and safety corrections, at no additional cost.

• Ensure equipment functions in conformance with manufacturer's specifications.

• Perform on-call remedial maintenance to restore equipment to serviceable condition within manufacturer's specifications.

• Provide annual training to Medical Maintenance personnel.

• PMI's and calibrations are to be performed annually.

• On-call remedial maintenance response time: within 24 hours (excluding weekends and holidays).

• Unserviceable units must be restored within 24 hours of arrival at the work site, with service completed within five working days.

• Contractor must provide a product certification annually, comparing the unit's accuracy to manufacturer specifications.

• Service reports are required for all services performed.

• Annual PMIs and calibrations.

• On-call service as needed.

• Location: Yokota Air Base, Japan.

• Contractor must provide written evidence of ability to obtain repair parts and that service personnel are trained and have 12 months of experience.

• Contractor must comply with HIPAA regulations and sign a Business Associate Agreement.

• Contractor shall provide a Contract Manager (CM) and alternate(s) on-site during work performance.

• Contractor personnel require authorization for access to Yokota AB.

• Government will furnish electric power and other utilities.

This document is a Notice of Intent to award a sole source, firm-fixed-price, non-personal services contract to Rees Scientific Corporation. The Defense Health Agency Contracting Activity (DHACA), Western Markets Contracting Division (WMCD) intends to procure annual service and preventive maintenance for the Rees Scientific Environmental Monitoring System.

Market research indicates that Rees Scientific Corporation is the only source capable of performing repairs or upgrades due to the proprietary nature of the equipment and software. No other company has authorized hardware or source code for software upgrades.

Services also include technical support, on-site refresher training, and validation documentation.
