Statement of Work (SOW) / Performance Work Statement (PWS)

This Performance Work Statement (PWS) outlines the requirements for non-personal services of an Advanced Practice Clinician (PA-C/APN) to support the mission of the Indian Health Service (IHS) within the Emergency Department (ED) SuperTrack program.

• Provide emergent/urgent Emergency Department/SuperTrack care services.

• Duties encompass emergent/urgent Emergency Department/SuperTrack services to IHS patients.

• Services are to be performed at the Navajo Area Indian Health Service (NAIHS), Shiprock Service Unit - Northern Navajo Medical Center (NNMC) in New Mexico.

• Period of Performance is flexible, allowing for short-term (13 weeks), long-term (1-year), and intermittent support.

• Performance will be evaluated based on a Performance-Based Matrix (Section 10.0).

• Key areas include Competency, Compliance, Patient Outcomes, Professionalism, Credentialing, Documentation, and Service Quality.

• Specific standards include 100% required tasks at 100% of required competencies, 100% compliance with IHS policies, and no reports of breached patient safety or infection control.

• Documentation must meet established IHS, Joint Commission, and CMS standards for timeliness, legibility, accuracy, content, date, time, signature, and designated profession.

• Place of Performance: Northern Navajo Medical Center, Shiprock, New Mexico.

• Period of Performance: Flexible, including short-term, long-term, and intermittent support.

• Contractor must be an Advanced Practice Clinician (PA-C/APN) with a minimum of two years of experience.

• Must possess a current, valid, unrestricted license in any U.S. state or territory.

• Mandatory certifications: BLS, ACLS, PALS, ATLS.

• Must meet health requirements, including a fitness for duty certificate and a clean contractor history regarding professional licenses, employment, and criminal actions.

• Required immunizations include Rubella, Mumps, Measles, Hepatitis B series, Chickenpox, and Tetanus Diphtheria (Td). Flu vaccination is required; COVID-19 vaccination is strongly encouraged.

• Must read, understand, speak, and write English and be respectful of local culture.

• Must possess basic IT skills.

• Must attend mandatory orientations and training.

• Background checks (Level I and Level II) are required.

• This is a non-personal services contract; the contractor acts as an independent contractor.

• Contractor must maintain liability insurance.

• Order of precedence for conflicts: Supervisor's directions, Position Description, PWS Exhibits, PWS Narrative.
