• Surveillance Methodologies: Details the primary methods for monitoring performance, including Random Monitoring, 100% Inspection, Periodic Inspection, and Customer Feedback.
• Surveillance Activities: Outlines specific activities such as reviewing monthly progress reports, task order progress reports, and performance results related to installation-accountable government property.
• Quality Assurance Documentation: Describes the process for determining performance, reporting results quarterly, and conducting reviews and resolutions with the contractor.

This QASP provides insight into how the government will measure and evaluate the contractor's performance under the NRESS III contract. Bidders can use this information to understand the government's expectations regarding quality, performance standards, and the surveillance process, which can inform their proposal development and understanding of contract management.

This document is Enclosure 8, an optional Cybersecurity Supply Chain Risk Management (C-SCRM) Questionnaire for Solicitation 80HQTR25R7002. It aligns with the NIST SP 800-161 template and is intended to simplify the fulfillment of C-SCRM requirements. The OCIO will accept either a plan that complies with NIST SP 800-161 or this questionnaire. Contractors with an existing NIST SP 800-161 compliant C-SCRM plan are not required to complete this questionnaire.

The questionnaire is divided into six sections:

• Section 1: Corporate Information: Collects company details, POC information, subsidiaries, and governing authorities.
• Section 2: Supply Chain Provenance: Inquires about suppliers, ownership verification, critical articles, threat analysis for distributors, foreign locations, and basic security requirements for NIST SP 800-171.
• Section 3: Supply Chain Management and Supplier Governance: Assesses identification of supply chain threats, supplier mapping, notification policies, written SCRM requirements, supplier reviews, and component reviews.
• Section 4: Physical and Personnel Security: Covers background checks for employees and procedures for preventing tampering of Information and Communications Technology (ICT) equipment.
• Section 5: Supply Chain Integrity and Resilience: Addresses documented processes for managing third-party products, component defects, integrity, end-of-life requirements, reuse/recycle of hardware and software, and supply chain resilience.
• Section 6: Inventory: Requires the completion of "6.1 - Hardware Inventory" and "6.2 - Software Inventory" to identify all contractor-owned hardware and software in use during the contract period. Specific instructions are provided for acquiring hardware and software, referencing the Commercial IT Request (CITR) process.

Bidders must complete this questionnaire or submit an equivalent NIST SP 800-161 compliant C-SCRM plan. Responses are required to be submitted at the start of phase-in. The C-SCRM plan, or questionnaire responses, should be submitted to the ICT/C-SCRM Service Element Lead at agency-dl-ocio-cys-cp-scrm@mail.nasa.gov, with a copy to the Contracting Officer. If a response to any question is "No," a summary explanation and risk mitigation plan must be provided.
