Evaluation Factors (Lowest Price Technically Acceptable - LPTA)

- Staffing Plan and Coverage Capability: Contractor is responsible for hiring, supervising, and managing personnel.
- Past Performance (Pass/Fail):
- Minimum three (3) relevant references within the last three (3) years.
- Relevancy: Healthcare staffing (specifically NP/PA-C or equivalent) in hospital, clinic, or FQHC settings. IHS, Tribal, or federal facilities are highly relevant.
- No history of contract termination, misconduct, debarment, or material deficiencies. A "Neutral" rating applies for no relevant history.
- Price (Pass/Fail):
