• Specific representations and certifications are required, including those related to small business status, telecommunications and video surveillance services, and other standard FAR provisions.

• Delivery is FOB Destination.

• Offerors are required to complete and return Provisions 52.204-24 and 52.204-26 with their quote, as failure to do so will result in the inability to make an award.

Statement of Work (SOW) / Performance Work Statement (PWS)

To provide a comprehensive Occupational Health Program for NASA employees at the Jet Propulsion Laboratory (JPL) in Pasadena, CA, focusing on maintaining and improving employee health, with an emphasis on prevention, diagnosis, treatment, and care of work-related illnesses and injuries.

• Furnish necessary personnel and services for an Occupational Health Program on a five-day per week basis.

• Provide health maintenance and job fitness examinations for NASA civil service personnel.

• Offer annual health maintenance examinations for newly appointed and transferring NASA employees at JPL.

• Provide follow-up and monitoring of abnormal findings from health maintenance examinations.

• Administer immunizations required by law and for official overseas travel.

• Review medical documentation for the NASA worker compensation program.

• Make available Health Education/Wellness services.

• Maintain accurate and complete medical records, ensuring confidentiality and compliance with relevant regulations.

• Period of Performance:

  • Base: April 1, 2026 - March 30, 2028
  • Option Year 1: April 1, 2028 - March 30, 2029
  • Option Year 2: April 1, 2029 - March 30, 2030
  • Option Year 3: April 1, 2030 - March 30, 2031

• Location: Contractor's local medical facility must be within 10 miles of 4800 Oak Grove Drive, Pasadena, CA.

• Maintain a local medical facility open five days per week, a minimum of forty hours per week during regular business hours.

• A physician and nurse must be on-site during hours of operation.

• Employ a systematic method for employee notification and appointment scheduling.

• Provide written reports for consultations and findings.

• Maintain medical records in accordance with NPR 1441.1 and other relevant regulations.

• Personnel must be qualified, with physicians Board Certified in Internal Medicine, Preventive Medicine, or Emergency Medicine, and licensed in California with experience in Occupational Medicine.

• Comply with all applicable Federal and State laws and regulations.

• Obtain necessary permits, licenses, and training.

• Ensure staff credentials are current.

• Medical records are the property of the Government and must be safeguarded.

• Privacy of Medical Information must be maintained in accordance with the Privacy Act of 1974 and HIPAA.

• Obtain laboratory services from a certified laboratory approved by NASA.

• Responsible for purchasing medical supplies, medicines, and equipment.

• Perform not more than 30 complete annual medical examinations per year.

• Notional estimate of 30 employees for whom services will be provided, subject to change.

• Services include annual health maintenance examinations, other examinations (Fitness for Duty, Return to Work, International Traveler, Disability report), and administration of immunizations.
