Statement of Work (SOW) / Performance Work Statement (PWS)

To ensure the provision of Occupational Therapy (OT) services to special education students identified or referred for services at the John F. Kennedy Day School on the Fort Apache Indian Reservation, in accordance with P.L. 108-446 (Individuals with Disabilities Education Improvement Act of 2004) and Individualized Education Plans (IEPs).

• Provide direct, in-person OT therapy to students.

• Conduct consultations with staff.

• Perform screenings and evaluation reports.

• Provide instructions and directions to teaching assistants.

• Participate in IEP meetings.

• Engage in parent contact regarding questions and concerns.

• Enter quarterly progress reports.

• Utilize the NASIS system for entering evaluation summary reports, present levels into IEPs, OT-related goals, and OT-related services.

• Maintain a direct service log for each student and visit.

• Meet with the Special Education Department at the contract's start and periodically for updates and progress discussions.

• Services must adhere to P.L. 108-446 and individual IEPs.

• Specific tasks include observing, screening, evaluating students, documenting referrals, attending multidisciplinary team meetings, implementing therapy plans, maintaining service logs, providing supervision to teaching assistants, identifying equipment needs, completing reevaluations and initial evaluations per BIE timelines, and submitting written progress reports quarterly.

• Period of Performance: Beginning August 2025, through May 2026, with a 4-year option.

• Place of Performance: John F. Kennedy Day School, Cedar Creek, Arizona.

• Contractor will be on-site 2 days per month to provide direct therapy to approximately 6 identified students needing OT bi-weekly.

• Key Personnel Qualifications: Occupational Therapist must have experience with school-age children, a Master's Degree in Occupational Therapy, be Certified Licensed in the State of Arizona, and hold National Certification in Occupational Therapy.

• Service Provider Responsibilities: Submit all documentation and reports to the Special Education Department, collaborate with the Special Education Coordinator to establish a schedule, and meet with the Special Education Department on the first day of work to organize service provision structure.
