Statement of Work (SOW) / Performance Work Statement (PWS)

To provide and install new office furniture in rooms 156, 157, and 158 at the Tohatchi Health Center.

• Provide all necessary labor, equipment, and supplies for the installation of workstations and furniture.

• All furniture must be wipeable and have micro-bacterial surfaces.

• Two U Shaped Desks with Hutch (including electric adjustable height desks, combo file drawers, and hutches with glass doors).

• One Sit-to-Stand Full Casing Desk (with electric adjustable height).

• One 4 Shelf Bookcase with Doors.

• One 29 1/2 inch Height Laminate Wood Door Storage Cabinet.

• One 2 Drawer Under Desk Mobile File Pedestal.

• Two 2 Drawer Lateral File with Hutch.

• One 42 inch Wide x 24 inch Depth Reversible Return.

• One PL142 Half Leg.

• One Under Desk Full Box/File Pedestal.

• Two Big & Tall Dual Layer Air Grid Mesh Manager Chairs.

• One High Back Multifunction Mesh Executive Office Chair.

• All hardware required for furniture connection and installation.

• A site visit to verify measurements, door locations, and power/data outlets.

• Adherence to 2018 Edition NFPA 99 and OSHA CFR A1926.

• Provision of worksite safety equipment and daily housekeeping.

• Proper disposal of debris at a state-certified site.

• Compliance with Gallup Service Unit Facility policies.

• Pre-Construction Control Risk Assessment (PCRA) approval required before work begins.

• Contractor(s) must sign in/out with Security.

• Submittals of all products and materials in scanned PDF format.

• Government reserves the right to inspect all work.

• Final acceptance based on acceptable final inspection.

• Only approved materials on submittal are to be used; no substitutions without prior written approval.

• All new materials to be installed per manufacturer's instructions.

• Work to be performed by a specialized contractor.

• Coordination with Tohatchi Health Center Facility Management staff is required, with 72 hours advance notification for issues/delays.

• Contractor responsible for obtaining necessary forms and permits.

• Contractor to verify existing site conditions and measurements.

• Facility staff will terminate electrical power; IT staff will provide data lines/jacks.

• Contractor to demonstrate full operation of all work performed.

• Provide a detailed project schedule.

• Work hours: 8:00 am - 4:30 pm, Monday-Friday (unless otherwise approved).

• From date of award through three months to complete the furniture installation.

• Authorized only after all work is complete, including punch list items and final inspection.

• Close out documents must include approved submittals and warranty letters.

• One year from date of acceptance.
