Statement of Work (SOW)

To provide professional, on-site document destruction services for Naval Health Clinic Oak Harbor (NHCOH) to securely shred and dispose of documents containing Health Insurance Portability and Accountability Act (HIPAA) and Personally Identifiable Information (PII) data.

• Provide secure, lockable bins in designated locations throughout the facility for staff to deposit HIPAA and PII related documents.

• Bins must be maintained by the vendor in a clean and operable state.

• Collect and securely shred bin contents no less than twice per month, specifically on the 2nd and 4th Tuesday between 0800-1400.

• Shredding services must be performed on-site by vendor service technicians.

• Vendor technicians must be in uniform, carry Picture ID, and provide a Certificate of Destruction upon completion of each shredding visit.

• Vendor shall recommend additional bins if current ones are filling faster than scheduled collection.

• Shredding to occur on the 2nd and 4th Tuesday of each month between 0800-1400.

• Bins must be maintained in a clean and operable state.

• Provide a Certificate of Destruction after each shredding visit.

• The service is for Naval Health Clinic Oak Harbor, Oak Harbor, Washington 98278.

• Specific bin locations will be determined by the NHCOH HIPAA Officer.

• Vendor must be National Association of Information Destruction Certified.

• Service Technicians must have current HIPAA training.

• Vendor must meet applicable Joint Commission Requirements for Recyclables and Bin Size.

• Vendor must be willing to use the government's iRAPT system for invoicing and payment.

• Vendor must enter into a Business Associate Agreement.

• Vendor must provide documented policies and procedures.

• Ability to shred various information sources (compact discs, x-ray films, credit cards, badges, etc.).

• Replace damaged bins at no cost.

• Two types of bins are specified: medium-sized (32 Gallons) for patient areas and small/executive style (16 Gallons) for offices.

• NHCOH has the right to change bin sizes and locations at no additional charge.
