Sources Sought Notice

This notice is issued by the Indian Health Service (IHS) to identify Indian Small Business Economic Enterprises (ISBEEs) capable of performing an Ophthalmic equipment services requirement for the Lawton Service Unit (LSU), which includes the Lawton Indian Hospital (LIH), Anadarko Indian Clinic (AIC), and the Carnegie Indian Clinic (CIC). The government is seeking to utilize the Buy Indian Act authority to give preference to Indian Economic Enterprises (IEE) where authorized and practicable.

• Service: 12-month Ophthalmic equipment services for service and repair.
• Equipment: Three (3) Carl Zeiss Cirrus OCT 5000 and three (3) Carl Zeiss HFA3 860.

Interested parties must submit a written capability statement demonstrating their company's capability, experience, and ability to perform the required services. Generic capability statements are not sufficient. Responses must include:

• Company Name

• Company Socioeconomic Status

• Company SAM UEI number

• Company contact information (point of contact, mailing address, phone, website)

• Contact for clarifications (name, phone, email)

• DUNS Number

• Cage Code

• Tax ID

• Date submitted

• Applicable GSA Schedule number or other procurement vehicle

• SAM registration status (registration on SAM.gov is required)

• Completed Indian Health Service Buy Indian Act Indian Economic Enterprise Representation Form (attached)

• Deadline: No later than 3:00 PM CST on March 5, 2026.

• This is NOT a solicitation for proposals. It does not obligate the Government to award a contract.

• Responses will not be acknowledged, and no feedback will be provided.

• A pre-solicitation synopsis and solicitation may be published following a review of responses.

This document is the "Indian Health Service Buy Indian Act Indian Economic Enterprise Representation Form." It is used by offerors to self-certify their eligibility as an "Indian Economic Enterprise" in response to a Sources Sought Notice, Request for Information (RFI), or Solicitation and resultant contract.

• Self-Certification: Offerors must attest whether they meet the definition of an "Indian Economic Enterprise" with "Indian Ownership" under the Buy Indian Act.
• Eligibility Period: The enterprise must meet the definition throughout the offer period, contract award, and the full contract performance period.
• Notification Requirement: Offerors must provide immediate written notification to the Contracting Officer if they no longer meet eligibility requirements.
• Required Information: The form requires the Offeror to provide:
  • Name of Federally Recognized Tribal Entity (if applicable)
  • Name of the 51% owner of the IEE Firm
  • Business Name
  • Certifying Signature
  • UEI Number
  • Print Name

This form is critical for any bidder seeking to participate in opportunities designated for or requiring self-certification as an "Indian Economic Enterprise" under the Buy Indian Act. Bidders must accurately complete this form to demonstrate their eligibility. Failure to meet the definition or provide accurate information can lead to disqualification or penalties. Successful offerors may also need to be registered in the System of Award Management (SAM).

Statement of Work (SOW)

To provide maintenance and repair services for Ophthalmic equipment at the Lawton Service Unit (LSU), which includes the Lawton Indian Hospital (LIH), Anadarko Indian Clinic (AIC), and the Carnegie Indian Clinic (CIC). The goal is to augment existing medical services and support health care within the hospital.

• Maintenance and repair services for Carl Zeiss Cirrus OCT 5000 and Carl Zeiss HFA3 860 equipment.
• Preventative maintenance following manufacturer guidelines.
• Unscheduled repairs, including OEM replacement parts, travel, and labor.
• Access to a service hotline for technical or application issues.
• Hardware and software updates as recommended by the manufacturer.
• Remote support for troubleshooting.

12 months from the date of award.

• Contractor personnel must comply with HHSAR and security clearance guidance, obtaining visitor passes.

• The service area is a tobacco/smoke-free environment.

• Contractor personnel must contact the designated government point of contact upon arrival and adhere to premises regulations, including potential escort by a Lawton Service Unit official.

• Contractor must not disclose any information concerning the operations of the Lawton Service Unit.

• Contractor shall furnish one designated Point of Contact (POC) for coordination.

• Contractor must provide a toll-free telephone number for service calls, answered between 8:00 am and 4:30 pm, Monday through Friday.

• Contractor shall furnish all personnel, materials, supplies, and equipment.

• Submit a service report and checklist (if applicable) to the Biomedical Engineering Department within 5 business days after services are rendered.

• Deliverables/Performance Matrix includes objectives for: OEM Planned Maintenance, Availability and Response for customer service concerns, Timely submittal of service reports, Notification to Biomedical Engineering Department, and Timely submittal of invoices.

• Acceptable Quality Levels (AQL) and Performance Surveillance are detailed for each objective, with non-conformance potentially leading to contract termination.
