This document outlines the specifications for the procurement of three (3) ophthalmic exam chairs for the Lawton Service Unit (LSU) of the Indian Health Service (IHS) in Oklahoma. • Supply three (3) Topcon OC-2400 Power-Recline Chairs with Motorized Tilt, or equivalent.

• Minimum specifications include:

  • Power-recline with motorized tilt function.
  • Minimum 500 lb (226.8 kg) weight capacity.
  • Hydraulic lift.
  • Foot pedal control for chair elevation.
  • Power Supply: 120V, 5A, 60Hz.
  • Articulating headrest.

• A minimum two-year warranty on all parts and labor is required.

• Period of Performance: 90 days from the date of award.

• Delivery, installation, and training coordination with the designated government employee.

• Delivery to Lawton Indian Hospital at 1515 Lawrie Tatum Rd, Lawton OK 73507.

• Deliveries must be completed by 4:15 pm; driver requires a valid ID.

• Contractor must provide one designated Point of Contact (POC) for coordination.

• Contractor must provide a toll-free telephone number for service calls, answered between 8:00 am and 4:30 pm, Monday through Friday.

• Any changes to specifications must be authorized and approved in writing by the Contracting Officer.
• Costs incurred by the contractor due to actions other than the Contracting Officer's are the contractor's responsibility.

This document is the "Indian Health Service Buy Indian Act Indian Economic Enterprise Representation Form." It is used by offerors to self-certify that they meet the definition of an "Indian Economic Enterprise" as required by the Buy Indian Act (25 U.S.C. 47) and HHSAR 326.601. This certification is necessary for responding to solicitations, Sources Sought Notices, or Requests for Information (RFIs) that are set-aside or awarded under the Buy Indian Act.

• Solicitation/Sources Sought/RFI #: Field to enter the relevant identifier for the opportunity.
• Project Title: Field to enter the title of the project.
• Date: Field to enter the date the form is completed.
• Representation: A checkbox section where the offeror must indicate whether they do or do not meet the definition of an "Indian Economic Enterprise" with "Indian Ownership."
• Name of Federally Recognized Tribal Entity: Field for the name of the tribal entity, if applicable.
• Name of the 51% owner of IEE Firm: Field for the name of the 51% owner.
• Business Name: Field for the offeror's business name.
• Certifying Signature: Field for the signature of the authorized representative.
• Print Name: Field for the printed name of the authorized representative.
• UEI Number: Field for the Unique Entity Identifier.

Bidders must complete and submit this form if the solicitation or opportunity requires it, particularly for contracts set-aside under the Buy Indian Act. Failure to meet the definition of an "Indian Economic Enterprise" or to properly complete this form may affect eligibility for award. Offerors must also be registered in the System of Award Management (SAM). False representations are subject to legal penalties.

Sources Sought Notice

This notice is issued by the Indian Health Service (IHS) to identify Indian Small Business Economic Enterprises (ISBEEs) capable of providing ophthalmic exam chairs to augment patient care delivery within the Lawton Service Unit (LSU), which includes the Lawton Indian Hospital (LIH), Anadarko Indian Clinic (AIC), and the Carnegie Indian Clinic (CIC). This is NOT a solicitation for proposals.

• The government is seeking capability statements from ISBEEs regarding their ability to provide ophthalmic exam chairs.

• Respondents must submit the Buy Indian Act Indian Economic Enterprise Representation Form (attached).

• Interested parties must submit a written response including specific company information (Name, Socioeconomic Status, SAM UEI, contact details, DUNS, Cage Code, Tax ID, GSA Schedule or other procurement vehicle, SAM registration status).

• Responses must be submitted via email to Mr. Jarrod Prince (jarrod.prince@ihs.gov) or Mr. Carroll Delaune (carroll.delaune@ihs.gov).

• The deadline for submission is May 19, 2026, no later than 1:00 PM CST.

• Maximum submission length is five (5) pages.

• The IHS must use the negotiation authority of the Buy Indian Act to give preference to Indian Economic Enterprises (IEE) where authorized and practicable.

• ISBEEs and other small businesses are encouraged to respond.

• This notice does not obligate the government to award a contract.

• Information provided is voluntary and will not be returned.

• No payment will be made for information provided.

• The government reserves the right to use non-proprietary technical information in future solicitations.

• No proprietary, classified, confidential, or sensitive information should be included in responses.
