Eligibility & Submission
This is issued as a Request for Quotation (RFQ). All potential quoters must be registered in the System for Award Management (SAM) for "All Awards" and include their Unique Entity Identifier (UEI) number with their quotes. SAM registration should include NAICS Code 621320. A Minnesota state optometry license is required. Resumes and certificates will be required from offerors. Quotes must be submitted electronically via SAM.gov, not directly via email, in PDF format only. Faxed and hand-delivered quotes will not be accepted. Contractors must comply with FAR 52.222-54, E-Verify Requirements.
Evaluation
Award will be made to the responsible Offeror whose quotation is determined to be the best value to the Government, considering price and past performance history. Failure to provide all required documentation detailed in Section L may result in elimination from consideration.
