This document is the "Indian Health Service Buy Indian Act Indian Economic Enterprise Representation Form." It serves as a self-certification for offerors to confirm they meet the definition of an "Indian Economic Enterprise" as required by the Buy Indian Act. This form is to be submitted in response to a Sources Sought Notice, Request for Information (RFI), or Solicitation.

• Eligibility Criteria: The form requires the offeror to self-certify that they meet the definition of an "Indian Economic Enterprise" throughout the offer submission, contract award, and contract performance periods.
• Notification Requirement: Offerors must notify the Contracting Officer immediately if they no longer meet eligibility requirements.
• Documentation: Contracting Officers may require documentation of eligibility, and awards may be subject to protest.
• Registration: Successful offerors must be registered with the System of Award Management (SAM).
• Legal Consequences: False or misleading information is a violation of law and subject to penalties.
• Representation Section: Bidders must indicate whether they meet the definition of an "Indian Economic Enterprise" with "Indian Ownership."
• Required Fields: Name of Federally Recognized Tribal Entity, Name of the 51% owner of IEE Firm, Business Name, DUNS Number, Certifying Signature, and Print Name.

This form is critical for bidders responding to opportunities that are set aside or prioritized under the Buy Indian Act. Bidders must accurately complete this form to demonstrate their eligibility and compliance with the Act's requirements. Failure to meet these requirements or accurately represent the business's status can lead to disqualification or penalties.

Statement of Work (SOW)

To establish a service agreement for the renewal of optometry service contracts for Daytona Optos Cameras at three Indian Health Service (IHS) locations: Clinton Indian Health Center, El Reno Indian Health Center, and Watonga Indian Health Center. The goal is to ensure extended coverage and prevent delays in patient care within the Optometry Departments.

• Provide on-site Emergency and Planned Maintenance support for all three Daytona Optos Cameras.

• Perform all planned maintenance as per the Optos System Service Manual.

• Repair systems, including parts, to a fully functioning state during Emergency Service events.

• Service hours are Monday through Friday, 8:00 a.m. to 5:00 p.m., excluding Contractor Holidays.

• Provide an on-site technician within 48 hours for service visits.

• Cover all parts, labor, travel and living expenses, and operating software for service and maintenance.

• Contractor must provide a letter of consent from Optos authorizing their services.

• Contractor must be available to service equipment in rural western Oklahoma.

• Contractor shall not provide parts, labor, or software for additional functionality without written agreement.

• Provide training to the Optometry/Government designee and Biomedical staff when necessary.

• Contractor must coordinate scheduled onsite visits with the designated government employee, providing at least a 24-hour notice.

• Deliveries must be made before 4:00 p.m. and drivers need a valid ID.

• Contractor must provide a toll-free telephone number for service calls, answered during working hours (8:00 am - 4:30 pm, Monday-Friday).

• Performance will be evaluated based on acceptability, fulfillment of responsibilities, adherence to standards and guidelines, quality control, quality improvement, and customer service.

• Base Period: 12 months (04/01/2026 - 03/31/2027)

• Four Option Years, each 12 months long.

• Locations: Clinton Indian Health Center (OK), El Reno Indian Health Center (OK), and Watonga Indian Health Center (OK).

• Contractor employees must comply with OPDIV DHHS/SE Region Human Resources, Security Clearance Guidance for Visitors and obtain a visitor's pass.

• The facility is a tobacco/smoke-free environment.

• Contractor personnel must contact the government designated point of contact upon arrival and are responsible for the security of all organizational information.

• Contractor shall not disclose any information concerning the operations of Clinton Indian Health Center.

• Required personnel, materials, supplies, and equipment are contractor-furnished.

Solicitation / RFP / RFQ

This document is a combined synopsis/solicitation for commercial items, issued as a Request for Quote (RFQ) # IHS1524836. It constitutes the only solicitation, and no written solicitation will be issued. The procurement is a 100% BUY INDIAN SET-ASIDE with a NAICS code of 811210.

The solicitation contains fifteen (15) line items, each requesting monthly service agreement support for Optos sites. The services include on-site Emergency and Planned Maintenance, performing maintenance per the Optos System Service Manual, and repairing systems with parts as required. The contractor must be authorized by Optos to provide services and must be available to service Daytona AF systems and equipment in rural western Oklahoma. Parts, Labor, Travel and Living, and Operating Software are covered under the agreement. The vendor must also provide an online site for DOT manifest training and records, and provide parts and operating software installations.

The period of performance spans five years:

• Base Year: 04/01/2026 – 03/31/2027
• Option Period One: 04/01/2027 – 03/31/2028
• Option Period Two: 04/01/2028 – 03/31/2028
• Option Period Three: 04/01/2029 – 03/31/2029
• Option Period Four: 04/01/2030 – 03/31/2031 The contract will be a firm fixed price award.

Quotes must be submitted electronically via email to Misti Bussell (misti.bussell@ihs.gov) no later than 03/18/2026 at 10:00 a.m. CDT. Evaluation will be based on Lowest Price Technically Acceptable, considering Pricing and Technical Capabilities. Pricing will be evaluated cumulatively over the 5 years. Technical capabilities will be assessed based on understanding of the Statement of Work and planned project execution.

This solicitation is a 100% BUY INDIAN SET-ASIDE.

All contractors must be registered in the System for Award Management (SAM) database. Quotes must include vendor name, Cage Code, DUNS number, Tax ID, prompt payment terms, delivery time, GSA contract number (if applicable), quote expiration date, warranty, total price per line item, total quote price, and sufficient technical documentation for evaluation. Failure to provide adequate technical detail may result in rejection. NO FAX QUOTATIONS will be accepted.
