Purpose & Scope
The primary objective is to establish a service agreement for the renewal of optometry service contracts for Daytona Optos Cameras at the Clinton, El Reno, and Watonga Indian Health Centers. This ensures extended coverage and prevents delays in patient care within the Optometry Departments.
The scope of work includes:
- Providing on-site Emergency and Planned Maintenance support for all three Daytona Optos Cameras.
- Performing all planned maintenance according to the Optos System Service Manual.
- Repairing systems, including parts, to a fully functioning state during Emergency Service events.
- Providing an on-site technician within 48 hours for service visits.
- Covering all parts, labor, travel and living expenses, and operating software.
- Obtaining a letter of consent from Optos authorizing the contractor's services.
- Availability to service equipment in rural western Oklahoma.
- Providing an online site for DOT manifest training for EVS staff and manifest records.
- Providing necessary parts and operating software installations.
