This is a combined synopsis/solicitation (RFQ No. 75H70726Q00060) for Outpatient Medical Nutrition Therapy (MNT) services for Native American patients served by the Taos-Picuris Service Unit in Taos, New Mexico. The goal is to prevent, delay, or manage individual diseases and enhance well-being through nutrition.

• Provide comprehensive nutrition assessments, develop and implement individualized nutrition care plans, and offer culturally appropriate nutrition education and counseling.

• Document all patient encounters, assessments, interventions, and outcomes in the Electronic Health Record (EHR) within 24 hours of the visit.

• Collaborate with interdisciplinary healthcare teams.

• Participate in population health initiatives and quality improvement activities.

• Maintain current professional credentials and complete required training.

• Provide consultation and/or in-service training on nutrition.

• Respond to notifications for patient visits within 24 hours.

• Technical Capability will be evaluated based on demonstrated ability to meet SOW requirements, including RD/RDN licensure, certifications, experience with chronic diseases, EHR documentation, and cultural competency.

• Past Performance will be assessed based on recency and relevance of prior work, with up to five (5) contracts/jobs to be provided.

• Price will be evaluated for reasonableness, completeness, and balance.

• Award will be made to the responsible quoter whose quote is most advantageous to the Government.

• This acquisition is a Total Indian Small Business Economic Enterprise (ISBEE) Set-Aside. Only quotes from ISBEE vendors will be considered.

• Minimum Qualifications: RD/RDN license, three (3) years of documented experience, advanced knowledge of nutrition science, and strong communication/cultural competency skills.

• Contractor will be provided with a workstation, computer, telephone, and office equipment.

• Medical liability insurance is required.
