This document, "Attachment 3 - Terms and Conditions," outlines the contract terms and conditions applicable to any order awarded under this solicitation. It supersedes all other terms and conditions and acceptance of an order signifies acceptance of these terms.

• Provisions: Includes instructions to offerors (52.212-1) and evaluation factors (52.212-2). The evaluation will be based on lowest priced, technically acceptable offer, considering price and technical acceptability.
• Pricing: Offerors must provide pricing as requested in "Attachment 1 – Schedule 70Z03826QJ0000203." Quantity price discounts and prompt payment discounts should be included.
• Technical Acceptability: Evaluated as "acceptable" or "unacceptable" based on the offeror's ability to meet SOW requirements and demonstrate understanding.
• Required Documentation: Offerors must submit overhaul turnaround time, OEM authorization (if applicable), a detailed explanation of any unmet SOW requirements, Certificate of Conformance (COC), and relevant certifications (FAA, EASA, NADCAP, USCG).
• Specific Clauses: Incorporates numerous FAR and HSAR clauses by reference, covering topics such as:
  • Security Prohibitions and Exclusions (FAR 52.240-91)
  • Sustainable Products and Services (FAR 52.223-23)
  • Contractor Employee Whistleblower Rights (FAR 52.203-17)
  • DEI Discrimination (FAR 52.222-90)
  • Buy American Act and Trade Agreements
  • Small Business Subcontracting Plans
  • Quality Assurance, Packaging, and Shipping Instructions

• Option for Increased Quantity: The USCG may require an additional ten (10) items for overhaul, up to a total of twenty (20) items, at the same price and turnaround time as the original order, within 365 days of the award date.

This document is a pricing schedule (in spreadsheet format) for the "Teardown, Test and Evaluation/No Fault Found", "Overhaul", and "Beyond Economic Repair / Scrap" of "Indicator, Blade" components. Bidders are instructed to fill in the yellow boxes with their pricing, lead time, and F.O.B. information. Any additional charges, fees, or prompt payment discounts must be clearly stated.

• Line Items: Includes "Teardown, Test and Evaluation/No Fault Found" (estimated quantity 1), "Overhaul" (quantity 10), and "Beyond Economic Repair / Scrap" (estimated quantity 1).
• Item Description: Specifies "Indicator, Blade" with NSN: 6685-00-900-8060 and Part Number: S6115-20520-003 (with alternate part numbers).
• Pricing Fields: Unit Price and Extended Amount are to be completed by the bidder.
• Lead Time: Must be specified.
• F.O.B. Information: Bidders must select "Destination" or "Origin" and include a shipping estimate if "Origin" is selected.
• Certificate of Conformance (COC): Bidders must indicate if a COC (IAW FAR 52.246-15) will be included.
• Option Clause: Bidders must indicate acceptance of FAR 52.217-6 for an option to overhaul ten (10) additional items within 365 days of award.

Statement of Work (SOW) / Performance Work Statement (PWS)

This Statement of Work (SOW) outlines the requirements for overhauling specific components for the United States Coast Guard (USCG) MH-60T aircraft. The primary goal is to return these components to an airworthy, Ready for Issue (RFI) condition.

• Component Overhaul: Overhaul of "Blade, Indicator" components (10 quantity, with 10 option quantity). The National Stock Number is 6685-00-900-8060, and the Part Number is S6115-20520-003.

• Corrosion Management: Components are expected to have corrosion due to operational environment (saltwater). Overhaul must include removal and treatment of corrosion according to OEM specifications and FAA Advisory Circular AC 43-4B.

• Teardown, Test, and Evaluation (TTE): Upon receipt, components will undergo TTE to determine their condition.

• Beyond Economical Repair (BER): If a component is determined to be BER, the Contractor will receive a TBD firm-fixed price. A failure data report is required for BER components, and disposition instructions will be provided by the Contracting Officer.

• Inspection and Repair: Components must be overhauled according to OEM functional performance specifications. This includes inspection, correction of deficiencies, replacement of missing or broken parts, and removal of foreign matter or corrosion.

• Components must be returned in an airworthy, RFI condition.

• Overhaul must comply with OEM functional performance specifications and relevant manuals.

• Corrosion removal and treatment must adhere to specified standards.

• Work is to be performed at the Contractor's facility or a USCG-approved facility.

• Delivery for Test and Evaluation is due fifteen (15) calendar days after receipt of components. The delivery date for Overhaul is TBD.

• Documentation: Requires a failure data report for each overhauled component, including NSN, P/N, S/N, date of overhaul, contract details, failure description, parts overhauled/replaced, and USCG's reported failure.

• Quality System: Contractor must maintain ISO 9001-2000 certification or equivalent.

• Certifications: Must maintain OEM certification for overhauled components. Requires DD Form 1574 or FAA Form 8130-3 tags, a Certificate of Airworthiness, and a Certificate of Conformance (COC) IAW FAR 52.246-15.

• Traceability: Requires complete unbroken history of ownership for replaced parts.

• Tracked Components: Components are tracked via the USCG's Asset Computerized Maintenance System (ACMS). Requires a Significant Component History Record (SCHR) and a Component Repair Record (CRR) to be maintained and submitted.

• Parts: Reused parts must be serviceable. Replacement parts must meet manufacturer specifications and have OEM traceability. Parts that have been cannibalized or severely damaged due to mishandling may be excluded from standard pricing.

• Audits: Subject to quarterly audits by ALC on Government Furnished Property.
