This solicitation is for a firm-fixed-price contingency contract to provide compounding and delivery of antineoplastic hazardous drugs from an FDA 503A compliant facility. The services are for patients receiving treatment and infusion services at the VA Southern Nevada Healthcare System (VASNHCS). Quantities provided are for estimation purposes only, and usage is dependent upon Government need.

• Compounding and delivery of hazardous medications for approximately up to 15 patients daily, Monday through Friday.

• Medications must have at least 12 hours remaining on Beyond-Use-Dating (BUD).

• Compounding, labeling, and BUD must comply with USP 797, USP 800, and Federal Law.

• Vendor must possess a certified 503A pharmacy with an active Nevada Board of Pharmacy license.

• Vendor must meet VA Pharmacy Benefits Management (PBM) Compounded Sterile Products Assessment Guidelines.

• Vendor must provide documentation of compliance with USP 797 and USP 800.

• Vendor must provide survey/inspection results, stability data, and certification reports upon request.

• Vendor must notify VASNHCS of anticipated shortages/recalls/discontinuations.

• Medications must be stored and used according to FDA approved labeling.

• Specific supplies for VASNHCS pumps and equipment are required (e.g., BD Alaris tubing, low protein binding filters).

• Offerors must submit quotes via email only to Leah Smith at leah.smith@va.gov.

• Offer Due Date: June 10, 2026, 11:00 AM PDT.

• Requests for Information (RFI) must be submitted two (2) business days prior to the closing date.

• Quotes will be evaluated on Technical and Price factors.

• The Government will rate quotes more favorably that meet or exceed requirements.

• Quotes must include manufacturer and part number for items, and any items not offered must be struck through.

• Only priced items will be considered for award.

• This acquisition is UNRESTRICTED.

• Delivery is FOB DESTINATION.

• Delivery to VA Southern Nevada Healthcare System, 119-Inpatient Pharmacy, 6900 North Pecos Road, North Las Vegas, NV 89086.

• Deliveries are required by 9:00 am the following business day after orders are submitted.

• STAT orders submitted after 12:00 pm PST may incur additional charges.

• Invoices must be submitted electronically via VA's Electronic Invoice Presentment and Payment System (Tungsten).

Statement of Work (SOW) / Performance Work Statement (PWS)

To secure a vendor for the compounding and delivery of antineoplastic hazardous drugs from an FDA 503A compliant facility on a contingency basis for patients receiving treatment and infusion services at the VA Southern Nevada Healthcare System (VASNHCS).

• Provide compounding and delivery of hazardous medications for approximately up to 15 patients daily, Monday through Friday.

• Medications must be compounded and delivered with at least 12 hours remaining on the Beyond-Use-Dating (BUD).

• Compounding, labeling, and BUD must adhere to USP 797, USP 800, and Federal Law.

• Vendor must be able to supply the entire list of compounded chemotherapy medications as needed.

• Orders placed by 12:00 PM PST will be delivered by 9:00 AM the next business day. Orders placed after 12:00 PM PST will be considered STAT and may incur additional charges for next business day delivery.

• Medications must be delivered to: VA Southern Nevada Healthcare System, 119-Inpatient Pharmacy, 6900 North Pecos Road, North Las Vegas, NV 89086.

• Specific supplies for VASNHCS pumps and equipment are required (e.g., BD Alaris tubing, 0.2 micron low Protein Binding Filter, BD PhaSeal CST device).

• Compliance with USP 797, USP 800, and Federal Law.

• Medications delivered with no less than 12 hours remaining on BUD.

• Adherence to delivery timelines (next business day by 9:00 AM for standard orders).

• Vendor must provide documentation of compliance with VA Pharmacy Benefits Management (PBM) Compounded Sterile Products Assessment Guidelines.

• Vendor must provide stability data and certification reports for sterile compounding areas upon request.

• Notification of anticipated shortages, recalls, or discontinuations.

• Vendor must possess a certified 503A pharmacy with an active Board of Pharmacy license valid for shipping to/within Nevada.

• Vendor must provide documentation of compliance with USP 797 and USP 800 standards.

• Vendor must provide survey/inspection results from governing bodies upon request.

• Contractor Point of Contact (POC) must be available Monday-Friday, 8:00 AM - 4:30 PM PST.

• Strict adherence to VA information security, privacy, and records management policies is required, including handling of Protected Health Information (PHI) and data breach protocols.
