This is a Request for Quotation (RFQ) for Pharmacy Labels and Supplies for the Northern Cheyenne Service Unit. The procurement is for commercial items and conducted under FAR Part 12. The government anticipates awarding a single Firm-Fixed-Price Purchase Order.

• Technical Questions Due: May 6, 2026, 2:00 PM MST. Answers will be posted by May 11, 2026.

• Quote Due Date: May 15, 2026, 3:00 PM MST. Quotes must be submitted via electronic mail in the specified format.

• Offer Acceptance: Offers must be firm for at least 120 days from receipt.

• Evaluation Basis: Lowest Price, Technically Acceptable (LPTA).

• Evaluation Factors: Technical Acceptability (meeting Technical Specifications, Past Performance) and Price.

• Award will be made to the technically acceptable offer with the lowest overall price. Discussions are not anticipated but may occur for minor clarifications.

• Set-Aside: 100% Total Small Business Set-Aside.

• NAICS Code: 323111 – Commercial Printing (except Screen and Books).

• Small Business Size Standard: 650 Employees.

• This RFQ does not obligate the Government to award a Purchase Order.

• Tamper resistant prescription pads: 10 pads required per pad (100 scripts/pad). Must meet state and federal regulations for prescription pads and include Service Units information.
• Padded poly mailers: Available in 6x10" and 8.5x12" sizes, with specified case quantities. These are for mailing prescriptions from the in-house pharmacy.
