Statement of Work (SOW) / Performance Work Statement (PWS)

To ensure dental equipment within the Phoenix Area Indian Health Service (PAIHS) facilities is fully functional, safe, and effective for its intended use through routine maintenance and calibration services.

• Provide annual preventative maintenance (PM) and repair services for dental equipment across approximately 140,000 Native American users served by IHS and tribally-operated facilities in Arizona, Nevada, California, and Utah.

• Services include scheduled routine maintenance (PM) and unscheduled repair/corrective maintenance.

• PM includes visual inspections, testing, calibration, and electrical safety tests, adhering to Original Equipment Manufacturer (OEM) specifications, ANSI/AAMI EQ89:2015, and Joint Commission requirements.

• Repair services are required to restore faulty equipment to proper operating condition.

• Contractor must furnish all necessary personnel, labor, facilities, equipment, parts, materials, and supplies, including travel, lodging, and replacement parts at an all-inclusive rate.

• Specific tasks for various dental equipment components are detailed, including but not limited to air compressors, delivery systems, dental chairs, handpieces, sterilizers, and X-ray units.

• Services must meet or exceed OEM requirements and relevant standards (ANSI/AAMI, Joint Commission, NFPA 99).

• A 100% compliance rate is required for all maintenance activities and frequencies.
