Statement of Work (SOW) / Performance Work Statement (PWS)

To provide uninterrupted medical services to pediatric patients at the Phoenix Indian Medical Center (PIMC) during a temporary HVAC system replacement in the Pediatric Clinic. This is an urgent/compelling request due to the temporary closure of main lobby restrooms.

• Provide a safe and clean ADA compliant restroom trailer.

• The trailer must include 2 private stalls and 1 ADA accessible stall.

• Features required: climate control, running water, granite countertops, porcelain sinks, wood floors, and modern LED lighting.

• A power source or generator is required for operation.

• The trailer must be equipped to handle up to 225 guests weekly.

• Minimum trailer dimensions: 16' wide, 28' long, 12' height.

• Requires a 20amp outlet and a 3/4" standard garden hose connection.

• Pumping service for waste tank (recommended once per week).

• Restocking of paper products and soap (recommended once per week).

• Rental start date: TBD (on or about).

• Estimated duration: Five (5) months, with an option to extend.

• Delivery Address: PIMC, Pediatric Clinic, 4212 N. 16th St., Phoenix, AZ 85016.

• Minimum Qualifications: Contractor must have at least 3 years of experience in portable restroom trailer rentals, setup, and servicing.

• Vendor is not responsible for clogged toilets; a service fee will be imposed if called out for this issue.

• Interior custodial services are not included; PIMC Environmental Services will clean the unit daily.

• Payment requests must comply with FAR 32.905(b) and the applicable Payment clause, typically submitted electronically via the Department of Treasury Invoice Processing Platform (IPP).

Solicitation Provisions

This document outlines the solicitation provisions that will govern the award of a contract. It details the terms and conditions that offerors must adhere to and that will be used by the Government for evaluation.

The Government will award to the responsible offeror whose offer is most advantageous, considering price and other factors. The evaluation will use a "Lowest Price Technically Acceptable" approach, with "best value tradeoff." Factors other than price are significantly more important than price.

• The Government will award based on the "best value tradeoff" approach.

• Price will be evaluated for reasonableness and overall value.

• Option prices will be evaluated, and unbalanced option prices may lead to unacceptability.

• A written notice of award or acceptance of an offer will result in a binding contract.

• The Government may accept an offer before its specified expiration time, even if negotiations occur.

Solicitation / RFP / RFQ

This document is a combined synopsis/solicitation for commercial items, issued as a Request for Quote (RFQ) with reference number IHS1524441. It seeks quotes for "PIMC Portable ADA Compliant Restrooms." The award will be a Firm Fixed Price contract. This is a Total Small Business set aside.

• The specific goods/services required are detailed in an attached Statement of Work (SOW).

• Offerors must propose on an all or none basis.

• NAICS Code: 562991 (Portable toilet renting and/or servicing).

• Award will be made using Acquisition Procedures in accordance with FAR 13.5.

• Contract type: Firm Fixed Price.

• Acceptance period: 120 days.

• Submission of Proposals Due Date: 25 MAR 26, 1:00 p.m. PDT.

• Written proposals are required; oral offers will not be accepted.
