• Remarks: Space for clients to provide narrative feedback, specific strengths, weaknesses, deficiencies, or overall comments.

Bidders must have their clients complete this questionnaire for past projects. The completed questionnaire will be submitted with the proposal as part of the Past Performance evaluation. The form guides the client on what information is needed to assess the contractor's performance and its relevance to the current opportunity. The government reserves the right to verify all information provided.
